Practice Nurse Job summary:
The Practice Nurse is an essential part of the Primary Care Team, helping to provide prompt, accessible and
professional care to patients registered at Framlingham Medical Practice. It is expected that the Practice Nurse
will use clinical judgement and assume professional responsibility and accountability in the delivery of evidence
based practice nursing care to the whole practice population.
To work proactively within the Nursing Team at the Framlingham & Earl Soham surgery sites as required
PRINCIPAL DUTIES AND RESPONSIBILITIES TO INCLUDE:
Practice Nurse duties & responsibilities:
 Identify, assess and diagnose patients with complex health needs across the practice population
 Support the introduction and use of technical and specialist nursing treatments for patients with complex
healthcare needs
 Support and develop programmes to improve health and well-being and meet the specific needs of
communities, groups and individuals
 Work with others to change policies to improve health and well-being and reduce inequalities
 Support the implementation and evaluation of evidence-based practice to improve the services offered
by the practice nursing team
 Support the team in quality assurance, development and clinical governance.
 Lead the practice in Infection Control, implementing policies, audits and attending Infection Control
Meetings and updates
Management of Respiratory Disease & Ischaemic Heart Disease:
o Maintain the disease registers
o Diagnosis, monitoring and development of individual management plans; agreeing these as
appropriate with the patient and other health professionals
o Identifying significant abnormalities
o Work collaboratively with the GPs to meet the targets for the Quality and Outcomes Framework
o Work within the local/BTS guidelines and patient group directions to manage patients with
Asthma and Chronic Obstructive Airways Disease as per the local CCG guidelines.
Therapeutic monitoring:
o Checking compliance with and adherence to appropriate treatments using an holistic patientcentred approach
o Recognising abnormalities
o Identifying the impact of treatment and implementing or recommending changes as appropriate.
Wound care and management:
o Assessing routine wounds including acute wounds, emergency minor injuries, burns scalds and
chronic wounds
o The management of tissue viability of all wounds
o Assessing and supporting good wound management in complex situations
o Supervising the Health Care Assistants in wound care
o Refer patients for Doppler assessment and to the Leg Ulcer Clinic when appropriate
Patient health checks:
o Working with patients to develop a management plan where health problems or potential health
problems are identified
o Identifying health trends and public health issues and working with the team and others to
combat these. Identifying significant abnormalities
o Obtaining appropriate information using a lifestyle questioning approach and identifying
appropriate health promotion issues
Risk assessment:
o Recognising issues and gathering sufficient information to refer (eg drugs; domestic violence;
child protection; vulnerable adults; senior patients; social problems)
Health screening:
o Considering the issues in the light of the practice population
o Recognising issues and gathering sufficient information to refer (e.g. women’s and men’s health,
sexual health, older people)
o Undertaking monitoring tasks (e.g. cytology as per the National Cervical Screening Programme,
chlamydia testing) and monitoring the outcome
o Providing in depth monitoring and advice as appropriate to the level of the post-holder’s
specialty and working with the patient in deciding on management plans.
Women's Health:
o Provide a comprehensive family planning service and sexual health advice
o Provide Condom Scheme to the under 24year olds ( male and female)
o Refer to the Lead nurse in Sexual Health or GP where appropriate, or to the Department of
Sexual Health
o Support in Breast awareness
Men's Health:
o Initiate opportunistic consultations with patients to cover issues of men's health, including
erectile dysfunction
o Administer GHNR injections where prescribed supporting the patient and family where
appropriate
Travel health:
o Provision of specialist and evidence based care
o Working with public health initiatives.
o To maintain competency in travel health demonstrating proficiency with the Travel Health
resources and knowledge of updates,
o To conduct a full risk assessment and offer a holistic approach to travel health maintaining
standards required by NaTHNac
o Administering vaccines, Malaria prophylaxis and comprehensive advice for patients prior to
travel in accordance with guidelines
o Attend annual training in travel health
o Provide an adult and childhood vaccination programme
o Work to Patient Group Directions and Patient Specific directions
o Working with public health initiatives.
o Supporting the team in achieving the immunisation guidelines and targets
First contact:
o Working independently or alongside the general practitioner and making decisions/
recommendations as appropriate.
Emergency Care:
o Provide initial emergency treatment in cases of Myocardial Infarction, Anaphylaxis, Head Injury,
Exacerbation of Asthma, Epilepsy, cardiac/respiratory arrest if no GP present
o Seek medical assistance for all other emergencies
o To attend annual CPR and anaphylaxis training
o
To support the team in maintaining and checking of the oxygen, emergency bag and equipment
on a monthly basis
Additional Clinical Procedures:
o Ear irrigation
o Suture removal
o Standard ECG and 24hr ECG/BP monitoring
o Phlebotomy
PERSONAL RESPONSIBILIITES
Personal/Professional development:
 Participation in an annual individual performance review, including taking responsibility for maintaining a
record of own personal and/or professional development.
 Taking responsibility for own development, learning, and performance and demonstrating skills and
activities to others. You should be able to work effectively both independently and as part of a team and
interact with patients and colleagues in a professional manner.
 Provide support for the Health Care Assistants and be the point of first referral for them.
 Attend statutory in-house practice training sessions and undertake appropriate training to support your
role.
 To work in accordance with the NMC Code: “Standards of conduct, performance and ethics for nurses
and midwives”.
 Maintain a high professional standard of nursing care, keep up to date to remain competent and provide
evidenced based nursing care relevant to the post.
 Participate in annual appraisals and be responsible for your agreed Personal Development Plan.
 Maintain an up-to-date portfolio and written professional development plan in line with the requirements
of registration with the NMC.
Other Tasks
 Attend regular Nursing Team Meetings, MDT, Macmillan Gold and other clinical meetings as required
 Ensure building security – have thorough knowledge of doors/windows/alarm.
 Ensure that all mandatory and clinical training is completed on time and kept up to date
 Any other tasks or duties allocated by Team Leads, GP’s or Practice Manager
IT/Computer
 Ensure that all Tasks, Notifications and scanned documents on S1 are processed promptly
 Ensure that emails on NHS.net are regularly read and processed as required

practice may only be divulged to authorised persons in accordance with the practice policies and
procedures relating to confidentiality and the protection of personal and sensitive data
 All staff are required to follow strict Information Governance Guidelines and undertake regular
Information Governance training
Health & Safety:
The post-holder will implement a full range of promotion and management their own and others’ health and
safety and infection control as defined in the practice Health & Safety policy and the practice Infection Control
policy and published procedures. This will include (but will not be limited to):





















Using personal security systems within the workplace according to practice guidelines
Awareness of national standards of infection control and cleanliness and regulatory / contractual /
professional requirements, and good practice guidelines
Responsible for the correct and safe management of the specimens process, including collection,
labelling, handling, use of correct and clean containers, storage and transport arrangements
Management and maintenance of Personal Protective Equipment (PPE) for the practice, including
provision, ordering, availability and on-going correct usage by staff
Responsible for hand hygiene
To closely follow infection control and clinically based patient care protocols, and implementation of
those protocols across the practice
Active observation of current working practices across the practice in relation to infection control,
cleanliness and related activities, ensuring that procedures are followed and weaknesses / training
needs are identified, escalating issues as appropriate
Identifying the risks involved in work activities and undertaking such activities in a way that manages
those risks across clinical and patient process
Making effective use of training to update knowledge and skills, and initiate and manage the training of
others across the full range of infection control and patient processes
Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of
hand cleansing facilities, wipes etc are sufficient to ensure a good clinical working environment. Lack of
facilities to be escalated as appropriate.
Safe management of sharps procedures including training, use, storage and disposal
Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and
safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to
responsible management
Actively identifying, reporting, and correction of health and safety hazards and infection hazards
immediately when recognised
Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and
hazards / risks in relation to other work areas within the business, and assuming responsibility in the
maintenance of general standards of cleanliness across the practice
Undertaking periodic infection control training (minimum annually)
Routine management of own team / team areas, and maintenance of work space standards
Waste management including collection, handling, segregation, container management, storage and
collection
Spillage control procedures, management and training
Decontamination control procedures, management and training, and equipment maintenance
Maintenance of sterile environments
Demonstrate due regard for safeguarding and promoting the welfare of children.
