participate in INCENTIVE PRACTICES on the property described in item number (2) on this form. The
landowner hereby authorizes the operator to receive incentive payments for implementing a Nutrient
Management Plan, Waste Utilization Plan and/or Pest Management Plan on the property described in item
number (2) on this form. The operator will be entitled to receive an incentive payment for each of the
practices listed below.
