HOW TO READ YOUR
EXPLANATION OF BENEFITS (EOB)
An EOB is a statement sent to members listing health care services you received, the amount billed and payment made.
Network Health issues an EOB after every claim has been processed, whether or not payment is due. It is important to
know this is not a bill you have to pay. Please refer to the example below for locating important information.
EOBs are also available online. For instructions on how to access them, go to networkhealth.com/eob
1 -SERVICE DATE
This is the date you received
health care services.
4 -PROVIDER
2 -PAID BY US
Network Health paid this
amount to the provider.
This is the provider who will be billing you directly.
3 - YOUR RESPONSIBILITY
This is the amount you owe.
5 - BENEFIT ACCUMULATORS
This is the amount applied toward your deductible
as of the date the claim was processed.
