New Diabetic REWARDS Program
We are replacing the 2015 Diabetes Eye Exam Incentive Program with our new Diabetic
REWARDS program.
To be eligible for the $50 gift card (not redeemable for cash) please complete these
three specifically selected care needs within calendar year 2016.
1. Get an A1C test completed with a physician order. A1C tests give you a good idea
on how well your diabetes treatment plan is working. The 2015 American Diabetes
Association Standards of Medical Care in Diabetes recommends an A1C goal of 7 or
less.
2. Get your annual diabetic eye screening with your eye care physician. Over time,
high blood glucose levels may hurt your eyes. There is no copay for a diabetic
screening only. The diabetic screening does not include a comprehensive eye exam
for prescriptions for eyeglasses/eyewear or other services.
3. Protect your kidneys either through a urine test with a physician order or by using a
specific blood pressure medication that helps to guard your kidneys from damage.
Here is our recommended plan of action for completing the Diabetes Bundle Incentive
Program:
1. Schedule an appointment and see your physician to discuss your diabetes treatment
plan.
2. Complete your lab tests after you obtain your physician’s order. You can have both
your A1C test and your urine test at the same visit.
3. Make an appointment to have your diabetic eye screening at one of the conveniently
scheduled clinics listed on the next page. Clinics are available between April 15 and
May 21 at various locations and convenient times. If these locations, dates or times
do not work for you, make an appointment to go to your eye care provider on a nonclinic day.
Your health care team will be monitoring these care needs through claims that have
been submitted. Once all three care needs are completed, a gift card will be mailed to
you within 60 days.
For your convenience, we have made arrangements with several local eye care
providers to hold eye exam clinics. You may choose to receive care at one of these
clinics or schedule an appointment with your own eye care provider.
