PRIISME Diabetes Project
Grey-Bruce Health Network & GlaxoSmithKline (GSK)

Brief Overview of PRIISME
The Grey-Bruce Health Network is pleased to
announce a local initiative in diabetes care.
The Grey-Bruce Health Network, along with
GlaxoSmithKline (GSK) pharmaceuticals, is
working towards standardizing patient care
and promoting best practice guidelines in
regards to diabetes education services for the
grey-bruce area. The goal of the PRIISME
program (Program to Integrate Information
Services and Manage Education) is to partner
with physicians and communities to assist in
the management of chronic diseases and to
promote the best possible treatment for these.
Through an integrated health-care approach,
PRIISME helps health-care providers support
patients suffering from asthma, diabetes and
chronic obstructive pulmonary disease
(COPD). It assists in providing for on-going
consultation, continuing education and other
initiatives to equip health care professionals
to be able to better educate patients and to
encourage the integration of care throughout
the community.
PRIISME helps to support efforts that enable
patients and their families to monitor their
own health and better manage their disease
thus improving quality of life.

What does this mean for
Grey-Bruce?
The Goal of the PRIISME Project is:
To provide residents with diabetes in Grey
and Bruce, with consistent evidence based
information and care, to be provided by
expert health care providers throughout the
community. Care will be equally accessible,
integrated and efficient with a focus on
creating an environment which empowers
people to take control of their health and
supports the concept of self-management. The
Grey Bruce Regional Diabetes Program will
endeavor to form community partnerships to
help improve health outcomes and prevent
complications for the people in our region.

What has been done so far?
Program Objectives are:

To promote consistency of
information for clients by
producing a standardized teaching
package to be distributed to
healthcare providers in
in our region.

To refine and standardize the data
collection tools presently being
used in our region.

To promote the use of clinical
practice guidelines, provide
educational opportunities and form
partnerships with physicians and
other healthcare providers in the
area.

To create a Regional Diabetes
Program that provides a
standardized approach,
consistency in information and
care and is based on best practice
guidelines.


Media Launch held
Two physician CME events have
been held thus far
Initial education with physician
offices on the PRIISME Project
and use of the information
packages
Education with community nursing
Standardized teaching packages
should be available soon for
hospitals, physicians offices and
healthcare agencies
Forms have been standardized for
Out-patient services
Work has begun on standard
teaching curriculum
In the process of…..
 Standardization of program
structure within the 3 hospital
corporations, thus promoting a
true Regional concept
 Forming a Stakeholders group to
involve our community partners
 Planning additional education
events
 Promotion of Clinical Practice
Guidelines and Train the Trainer
sessions for physician offices
 Ongoing evaluation
Promote diabetes education
services and advocate on behalf of
our clients.
