About us Our philosophy The West Hampshire Community Diabetes Service offers diabetes care and education to healthcare professionals throughout West Hampshire at a variety of community hospitals and GP surgeries. We offer a varied portfolio of education for patients and healthcare practitioners. Wherever possible we aim to support patients to improve self care. Tadley Whitchurch People with diabetes tell us that diabetes is both a daily challenge and a life long journey. Evidence supports the use of structured group education programmes for patients with Type 1 and Type 2 diabetes. Increasingly, people with diabetes are seeking the tools and knowledge to improve their own diabetes management, empowering them to make choices and individualise their care. We share our specialist diabetes knowledge with health care practitioners working in the community which then ensures that people with diabetes have access to consistent, high quality care where and when you need it.
