Genetic Studies (if applicable):
In order to interpret the results of genetic research, we need to have correct information about
parents. Sometimes the research shows new information about birth parents. This could
happen in the case of an adoption or a mistake in the identity of a mother or father. This
information will not be given to anyone, including yourself or family members.

Future use of tissue/DNA samples (if applicable):
In order to preserve a valuable resource, your [tissue/DNA] samples may be stored at the end
of this research project. It is possible that these samples may be used in a future research
project. This project will be related to [childhood cancer, asthma, etc.] as determined by a
research ethics committee. Any future research would have to be approved by a
Research Ethics Board (REB).

studied, i.e. cancer, arthritis, etc.) research project without contacting me again, but
