Details of services – M/s Super Religare Laboratories Ltd., (SRL)
SCOPE OF SERVICES & FEES
SRL hereby undertakes to extend its Health Packages and Pathology Tests to the Beneficiaries
at pre-decided rates (mentioned below):
A. Offer on Pathology: SRL shall give 20% flat discount on MRP on Routine and Specialized Test
to Beneficiaries across all India Wellness centers.
B. On Site Camp: On site camps across all ICSI locations in India after mutual consent of both
Parties.

D. Modus operandi:
(i) Beneficiaries would walk-in SRL Wellness Centers (As per list given in Annexure- B) for Health
Packages and pathology Tests. Reports will be generated and given to the Beneficiaries directly
as per Turn Around Time (TAT).
(ii) Beneficiary shall carry an Identity Card issued by the ICSI, which SRL shall use as Proof of
Identification. In respect of the family members of the Beneficiaries, the said member will be
required to carry the photocopy of the Identity Card of the Beneficiary. The said copy should be
signed by the beneficiary mentioning therein the particulras of the family member namely their
name, age and sex.
(iii) SRL and its associates would be generating a Client Code for ICSI. The reporting of the
investigations carried out by SRL will be done under this Client Code only.
