Commonwealth Solar II Rebate Program: Application Cover Page Note: Expedited Installers must upload required documentation directly into PowerClerk. All files uploaded in PowerClerk must be PDF files 5MB or smaller. Non-Expedited Installers should email the required documentation to [email protected]. Host Customer Name: Application Cover Page (optional): Fill out completely. Rebate Application: Fill out completely, sign, and include ALL pages. Electric Bill: The Electric Bill should clearly note the utility company, customer rate code, and full site address. If site address on bill does not match the site address, please clarify (i.e. Unit number or different town name). Aerial Image, Photos, and Shading Requirement: Include site photos, an aerial photo (identify the system location), and a shading analysis if there is ANY shading on site. Copy of Executed Contract (optional) between Host Customer and Third-Party Owner (for Third-Party Ownership projects only) Adders MA Company Components Adder: Verify components listed in application meet program requirements and the adder box is checked in PowerClerk. Proof of Primary Residence: Include a valid driver’s license, voter registration, Massachusetts State issued ID, or vehicle registration. Moderate Home Value Adder: Copy of most recent property tax bill or assessment listing the total assessed property value, address, and host customer. Moderate Income Adder: Letter of income eligibility from Spillane Consulting Associates (SCA). Natural Disaster Relief Adder: Include FEMA, MEMA, or insurance company documentation for natural disaster damage. Include Attachment D for third-party owned projects. Optional at rebate application back-up documentation submittal: Energy Efficiency (optional): Evidence of energy audit, scheduled appointment, or building permit for construction of site issued after 10/6/2008 for a residential building or 3/1/2009 for a commercial building. Application Notes (please provide any additional information that may be helpful in the application review process, i.e. trees will be trimmed/removed by homeowner; address clarifications) Application Cover Page Commonwealth Solar II: 2014-CSII-Version 20.0 Effective 10-07-2014 Page 1 of 3 Commonwealth Solar II Rebate Application Informational Form In order to expedite the rebate application review process, the MassCEC has compiled this informational form to highlight exactly what is needed in the application review process and to note common errors found. Commonwealth Solar II Rebate Back-up Documentation Notes: Application Cover Page (optional) Fill out completely and note anything that might be confusing or helpful to know during the review process. Rebate Application Expedited Installers must provide a copy of the Version 20 PowerClerk application. Non-Expedited Installers must provide a copy of the Version 20, Attachment A-1, or A-2. On all applications, verify that the System Owner, Host Customer (if different), and Primary Installer have signed in the correct signature field on the application. Make sure all sections are filled out, including the electric rate code. When submitting the application backup, include all pages of the application, not just the signature page. Electric Bill The electric bill must show the utility rate code, full site address, and the utility company name. If the account address or the name on electric bill is different than the site address or Host Customer name listed on the Rebate Application, please offer an explanation on the Application Cover Page or in the installer notes section in PowerClerk at the time of back-up documentation submittal. For new construction projects, the Host Customer must offer a work request number or demonstrate that a utility service account is in the process of being created. Photos Please submit an aerial photo as well as additional site photos if there is shading. The aerial photo should note the PV array location as well as any changes that have been made to the site. If the aerial photo is out of date, please indicate so on the Application Cover Page or in the installer notes section in PowerClerk. Copy of Executed Contract (optional) between Host Customer and Third-Party Owner It is requested, though not required, that Primary Installers/Integrators submitting applications for Third-Party Ownership projects also include a copy of the executed contract between the Third-Party Owner and the Host Customer with the application documentation. This is for data collection purposes only. MassCEC is not responsible for verifying that the information included in the contracts matches the information provided in the Application and other project documentation. Shading Analysis (Highly Recommended) A copy of the shading analysis is highly recommended at the time of application to ensure the system will meet the 80% of optimal output requirement outlined in the Minimum Technical Requirements (Attachment C). Please make sure that the analysis considers the full tree line. If there is a reason that part of the tree line has been excluded (i.e. the homeowner is planning on removing trees), please explain this on the Application Cover Page or in the installer notes section in PowerClerk and note that an updated analysis demonstrating that the system will meet the 80% of optimal production requirement will be submitted at project completion. MassCEC reserves the right to request a copy of the shading analysis at any time. Solmetric SunEye, Solar Pathfinder, Wiley ASSET software or other MassCEC approved shading analyses will be accepted. Please note that additional information from PV Watts may be required if Wiley ASSET is being utilized. Please see the program manual for more information. Application Cover Page Commonwealth Solar II: 2014-CSII-Version 20.0 Effective 10-07-2014 Page 2 of 3 MA Company Components Adder Verify components meet the requirements (the components listed in the application must be eligible to qualify for the adder) and that the adder checkbox is selected in PowerClerk. If using GreenRay equipment be sure to input the equipment appropriately in PowerClerk, and if using Panel Claw racking, be sure to provide notes accordingly (for Expedited Installers only). Proof of Primary Residence Proof of Primary residence can be a current voter’s registration card, a customer driver’s license showing installation site address, a Massachusetts State issued identification card showing the project site address, or a vehicle registration (of the Host Customer’s primary vehicle). If submitting a driver’s license, please obscure the license ID number. The address on the documentation should match the project site address listed on the application. Moderate Home Value Adder Include a copy of the Host Customer’s most recent tax bill or property tax assessment. The documentation must include the total assessed property value (land and building). If the name listed on the tax bill or property assessment does not match the Host Customer’s name, please explain the discrepancy on the Application Cover Page or in the installer notes section in PowerClerk. If the building is new construction, MassCEC will accept an appraisal of the new construction building and property. Moderate Income Adder Include a letter of income eligibility from Spillane Consulting Associates (SCA) for either the individual or domestic unit status. Natural Disaster Relief Adder Include FEMA, MEMA, or insurance company documentation for natural disaster damage. Include Attachment D for third-party owned projects. Common Rebate Application Errors: 1. Application: The Version 20 Application was not submitted, or the System Owner / Host Customer / Applicant did not provide a signature, or the signature is in the incorrect field. 2. Electric bill: Discrepancies exist between the account address and the site address on the Rebate Application. For projects receiving the Moderate Home Value or Moderate Income Adder, the page of the electric bill that shows the utility rate code is missing. 3. Photos: The installation site in the aerial photo and/or site photos is not clear or appears to be shaded. 4. Shading Analysis: Trees or other features are excluded without explanation. TSRF of one or more arrays is less than 80% of optimal (In this case, we can accept a weighted break out). 5. Residential Adder documentation: There is missing or contradictory information listed for Moderate Income or Moderate Home Value adder. 6. Host Customer Sector: The sector is entered incorrectly. Please note that Residential Solar Leasing applications should choose “Commercial” as the Host Customer Sector, and “Residential Solar Leasing” as the Host Customer subsector.
