HIGH PLAINS RESEARCH NETWORK
Position Description: Quality Improvement Coach
NATURE OF WORK:
The High Plains Research Network (HPRN) is an integrated research network of primary
care practices, hospitals, and communities in northeast Colorado. The goal of the HPRN is to
conduct clinically important primary care research in and with rural communities. The academic
and administrative home of HPRN is the Department of Family Medicine, University of Colorado
at Denver.
HPRN is partnering with Health TeamWorks to bring its Patient-Centered Medical Home
(PCMH) quality improvement research program to primary care practices in northeast Colorado.
Funding for this quality improvement research is provided by a grant from the Colorado Health
Foundation.
As a member of the High Plains Research Network (HPRN) Practice Transformation
Team, the Quality Improvement Coach (QIC) will hold a full-time position. The QIC will be
responsible for the implementation and data collection of the quality improvement research
program as specified by the protocol in a caseload of practices in northeast Colorado. The QIC
utilizes an evidence-based approach to facilitating quality improvement work both directly in the
practices and within this organization. The QIC maintains accountability for practice measure
reporting as well as individual assessment of practice progress toward desired transformational
change.
SUPERVISION RECEIVED:
This position will be under the general supervision and authority of the Director of the
High Plains Research Network. Training of the QIC on the PCMH protocol will be led by Health
TeamWorks, with support from the HPRN team.

SUMMARY OF DUTIES AND RESPONSIBILITIES:
The QIC is responsible for implementing the quality improvement protocol with practices
to facilitate redesign efforts, supporting their achievement of improved patient outcomes,
increased patient, provider and health care team satisfaction, and efficiency within the practice;
collect and analyze data about redesign efforts; and feedback to the practice results of data
analysis and redesign of effort when needed.Essential duties include:

Establish and maintain a supportive, collegial role with HRPN practices engaged in
improvement activity.
Using a standardized approach based on Health TeamWorks’ PCMH research protocol:
o Responsible for baseline data collection and assessment of practices.
o Introduce and educate incoming practices to key principles related to
implementation, foundational concepts (such as the Planned Care Model and the
Model for Improvement), and accountabilities.
o Utilize associated Change Package and related high-leverage changes to
support practices’ establishment of improvement aims and tests of change.
o Assist the practice in maximizing use of available health information technology
resources in order to accomplish improved patient outcomes and increased office
efficiency focusing on such areas as:

ƒ Population management and registry use/incorporation of registry
ƒ Planned care
ƒ Coordination of care
ƒ Goal setting and action planning with patients
ƒ Reporting of measures
ƒ Patient safety
o Assess and document practice progression using such tools as the QIC Report
and field notes.
o Review data monthly with individual practice improvement teams.
o Prepare practices for transition to next level as warranted by situation.
Facilitate efficient, effective improvement team meetings. Role model these skills in
order that improvement team leaders will also be able to effectively manage
improvement team meetings with increasing independence over the course of time.
Regularly maintain databases directly associated with role, such as the IPIP Database.
Link practice to available external resources, such as consultants and group learning
opportunities, to meet specific needs beyond those met by the QI Coach’s role.
Participate in Practice Transformation Team Monthly Practice Data Review.
Participate in the planning and hosting of program’s Learning Collaboratives
Effectively collaborate within and across PCMH teams.
Work with HPRN and the Practice Transformation Team in summarizing and
disseminating experience-related learning by way of team updates, written
reports/articles, and/or presentations as called for by directors.
Maintain collegial relationships with HPRN’s and Health TeamWorks’ external partners
including, but not limited to, collaborating organizations, funders, and vendors.
MINIMUM PREPARATION FOR WORK:
• Minimum of B.A/BS in health care or related field
• Course work in Quality Improvement preferred and/or facilitation skills
• Computer Knowledge:
o Excellent computer skills particularly related to Microsoft applications including
Word, Access, Excel, PowerPoint and Outlook
o Comfort with learning to use unfamiliar applications or databases
o Ability to teach practices how to maximize technology
• Communication Skills:
o Strong interpersonal and communication skills a must
o Excellent verbal, written, presentation, and group facilitation skills required
o Strong critical thinking skills necessary to evaluate and respond to team progress
and related team dynamics
• Reside in northeast Colorado
• Ability to travel within the northeast HPRN region and to Denver on a regular basis
• Ability to work independently and remotely
• Strong multi-tasking and organizational skills essential
PREFERRED QUALITIES:
• Five years experience in health care
• Experience in primary care practice setting
• Experience with quality improvement activities
• Course work in Quality Improvement

SALARY RANGE:
$40,000 - $50,000, commensurate with experience. The University of Colorado Denver offers a
