About Sharon S. Richardson Community Hospice Sharon S. Richardson Community Hospice is Sheboygan County’s only locally-owned, community-based hospice provider. We are a nonprofit organization dedicated to providing high quality, compassionate end-of-life care. Our home hospice and palliative care services are provided wherever a patient calls home within Sheboygan County and surrounding communities. The Sharon S. Richardson Community Hospice inpatient center in Sheboygan Falls is the only dedicated hospice facility in the Wisconsin East Central Lakeshore Region. Skilled nursing care is provided in the hospice center for patients in need of general inpatient level services. Respite care is also provided in the center to give families a rest from caring for their loved one at home. Experience Confidence Your loved one deserves the best. At Sharon S. Richardson Community Hospice our goal is to help you feel confident that your hospice team is working to provide the best hospice care possible. We offer: • An on-site Medical Director who is certified by the American Board of Hospice and Palliative Medicine and the American Board of Family Medicine. • Evening and weekend tuck-in services for home hospice patients. Whether in a facility setting or a private home, we understand that everyone could use a little extra help at night. We’ll be there for you. • Same-day consultations and admissions. • Accessible staff. Our own staff take your calls 24 hours a day. • Certified End-of-Life Nursing Education Consortium (ELNEC) trainers on staff. We are leaders in hospice training and will help you feel confident in caring for your loved one. If your loved one resides in a nursing home or other care facility, our trainers will educate the facility staff to provide a higher level of care. Sharon S. Richardson Community Hospice is consistently rated higher than the State and National Average of “Excellent” scores for the “overall quality of care that primary caregivers (usually a family member) felt was given to a patient.” Professional Support Your hospice team includes: • Our Hospice and Palliative Medicine Certified Medical Director • Your personal physician(s) • Nurses and aides trained in hospice • Social workers • Spiritual care coordinators • Bereavement coordinator • Specially trained hospice volunteers Your hospice team will work in collaboration with you and your family members or caregivers to create a personalized plan of care. You will have access to your hospice team 24 hours a day, 7 days a week. You choose the services you want and how often you want them. Free Consultations, Any Time, Any Place We are experts in end-of-life care and our desire is to give you and your family the best experience. Our staff is available 24 hours a day to answer your questions. You do not need a doctor’s referral for a consultation. We’ll even come to you.
