DISHLER AFFILIATED DOCTOR CO-CARE INFORMATION PRE- OPERATIVE EXAM Patients will need a pre-operative examination done between 2 days and 90 days before surgery. This testing should include a complete ocular health screening (Ocular/Medical History, Entrance Testing, IOP, SLE, DFE) and refractive data (Past SRX, Dry Refraction, Cycloplegic Refraction). If the pre-op is being done in the same week as surgery, please use Tropicamide for the Cyclo exam so the patient will not still be dilated on surgery day. If you use electronic records for pre-op exams we request that your data be transferred to our standard pre-op form so the doctor is able to review and sign it to ensure accuracy. Data on both eyes is always requested, even if the patient is only having surgery on one eye. Please call our office ahead of time if there is anything unusual with the patient’s ocular or medical history that may require specific education or a second opinion before surgery day. Once the pre-op is complete please fax it to our office at 303-793-3008. POST- OPERATIVE EXAM Our standard post-operative exam schedule is 1 Day, 1 Week, 6 Weeks, and 6 Months post-surgery. Patients can be seen as much as needed for post-operative related complaints within the first year after surgery. We encourage patients traveling from long distances to stay overnight in Denver and see us for their 1 Day post-op visit. If a local patient will be seen in your office for their 1 Day post-op we request that it be scheduled first thing in the morning while our Surgical Center is set up for any flap complications. If you use electronic records for post-op visits these do not need to be copied to our form before being sent to our office. We request that you fax each post-op exam as the patient comes in so Dr. Dishler can review the patient’s progress. Feel free to call our office if you have concerns about a patient’s recovery or if you would like them to be evaluated by Dishler Laser Institute. Dr. Dishler is available by appointment to meet with any patient before or after surgery. CONSULTATIONS Co-Care patients only have to make one trip to our office on surgery day. Corneal testing including Pachymmetry, Topography, and Wavefront Aberrometry can be performed on the same day as surgery. Patients that are interested in coming in ahead of time to make sure they are candidates, meet our doctors and tour our facility are welcome to schedule a free consultation. We also offer free Corneal Topography testing for any patient that you feel may have Keratoconus. If one of your patients visits us for a consultation you will receive a faxed report detailing the exam and they will be sent back to your office for their pre-operative exam. 1 CONTACT LENS REMOVAL We recommend that all contact lenses be removed for at least 3 days prior to a LASIK consultation or a pre-op exam. Before surgery we recommend that soft contact lenses be removed for a minimum of 1 week (longer in the case of dry eye or refractive instability). Rigid Gas Permeable/Ortho-K lenses should be discontinued at least one month per decade of wear. DRY EYE TREATMENTS In order to help keep the cornea well hydrated after surgery, a One Week Temporary Collagen Punctal Plug will be inserted into the lower tear duct for each patient on surgery day. Patients are educated with our “Dry Eye Form” that additional Temporary or Permanent Punctal Plugs may be recommended in the recovery period and that the cost of these are not included in the original price of surgery. We often use mild steroids/Restasis/Xiidra for any patient who experiences significant dry eye before surgery or is at a high risk for developing increased dry eye after surgery. Patients can use Restasis/Xiidra up until surgery day and then resume treatment again at 1 week post-op. SURGERY DAY Surgeries are performed in our office on Tuesdays and Thursdays, morning and afternoon. Patients are given an arrival time and can expect to be in our office for 2-3 hours total. We offer a mild oral sedative before the procedure and patients will need a driver to take them home after surgery. The Operative Report will be faxed to your office after your patient’s procedure and our office will leave a message with your front desk if a Bandage Contact Lens is placed on one of your patients for removal at their 1 Day post-op exam. When a patient calls to schedule surgery they will be asked who their Co-Care Doctor is and be instructed to schedule a pre-op exam if it has not already been completed. Patients will receive a packet of information in the mail a few days before surgery that includes the consent forms, surgery day instructions, and steroid prescriptions. We will provide the antibiotic eye drops for all patients, but everyone will be given a prescription for 5ml Prednisolone and 10ml FML to fill and use post-operatively. Patients will also be called the day before surgery to confirm arrival time and review final surgery day instructions. CO-CARE FEES We request that co-care doctors collect their own pre and post-operative fees. This allows you to receive immediate payment for your services and set your own fees based on what you feel is appropriate for each patient. When patients call our office inquiring about surgery, we quote the total fee listed on our pricing sheet, noting that this includes all pre and post-op care which may vary slightly with each eye care provider. If you have a patient scheduled for surgery we will send you a fax one week ahead of time as a reminder to confirm that the pre-op has been completed and arrangements for collecting fees have been made. 2 CANCELLATION POLICY There is no down payment required to schedule surgery. Patients are also allowed to reschedule surgery at any time with no fee. If a patient needs to cancel surgery, there is no fee as long as it is done more than 7 days before the scheduled surgery date. If the surgery is cancelled within 7 days and the patient does not want to reschedule, there is a cancellation fee of $300. FINANCING Dishler Laser Institute participates in financing through Care Credit. Applications are available at www.carecredit.com and through our website at www.dishler.com. Patients can also call our office to get pre-approved before coming in for a consultation or surgery. If a patient has difficulty getting approved we recommend that they call our office to have the application run again, as there is a higher approval rate directly through our office. ENHANCEMENTS Our Enhancement Policy is included on a separate sheet in our Patient Information Packet for each patient to review before surgery. With our newest laser technology our enhancement rate is currently less than 2%. If you have a patient that you feel would benefit from an enhancement, please fax the patient’s post-operative exams to our office to be reviewed by Dr. Dishler. If the patient is approved they will be asked to complete a full pre-operative exam and then schedule surgery. Enhancement procedures are performed the last Tuesday of every month and we recommend doing one eye at a time in the rare case that an enhancement is required on both eyes. A Bandage Contact Lens is inserted on all eyes following an enhancement surgery and it can be removed at the 1 Day post-op visit. The earliest an enhancement would be considered for LASIK is 4-6 months post-op, and for PRK and SMILE is one year. When patients are scheduled for an enhancement, their final candidacy is dependant upon testing completed in our office on surgery day. Dr. Dishler will review the appearance of the flap edges and the post-operative corneal testing to make sure that a second surgery is in the patient’s best interest. Patients are welcome to have the enhancement evaluation completed at our office before surgery day to make sure they are a candidate, or on the same day as surgery for convenience. EMPLOYEE DISCOUNT Any full-time employee at your office that works directly with potential surgery patients may be eligible to have surgery at Dishler Laser Institute at a discounted Surgical Fee. All pre and post-operative care must be done at your office at no charge. If you have an employee that is interested, please fax their pre-operative examination to our office for Dr. Dishler to review for candidacy. We encourage the Co-Care Doctor or other staff members to come along on their employee’s surgery day to see what patient’s experience and also watch the procedure if desired. 3 PATIENT INFORMATION PACKETS Patient Information Packets are available so that patients are well educated before having surgery at Dishler Laser Institute. They are located within walking distance of our office and offer a complimentary shuttle for patients to and from our office on both surgery day and the 1 Day post-op visit.
