Afasic is the UK charity that helps children and young people affected by the hidden disability of speech,
language and communication impairments, working for their inclusion in society and supporting their parents
and carers. Across the UK there are over one million children and young people affected by this hidden, but
very real, disability.
Afasic works to empower parents and carers to advocate for their child; supports and develops the
communication, social and life skills of children and young people with SLCI; increases the skills and knowledge
of professionals; and actively lobbies government for better resources and specialist services.

have read this statement and want the charity or Community Amateur Sports Club (CASC) named above to
reclaim tax on the donation detailed below, given on the date shown. I understand that I must pay an amount
