2014 HFPN Physician Incentive Structure
The HFPN Board of Trustees recently approved of the 2014 HFPN incentive model. Incentives will be awarded
based on a combination of network and individual physician performance related to the following metrics:

Cost of Care* (measured at the Network level)
30-Day Readmission rate (measured at the Network level)
ED Visits/1,000 patients (measured at the Network level)
Patient Satisfaction (measured by individual physician performance)
Network Participation (measured by individual physician performance)
*Per member per month/episode treatment group
Payment Criteria
Payment is based on a 100 point scale. Physicians accumulate points by achieving targets for the metrics above,
and each physician will receive a percentage of the maximum payment based on their point total.
The chart below outlines the 2014 incentive model:
*Incentive scoring for Cost of Care, 30-Day Readmissions Rate, and ED Visits/1000 metrics will be determined by Network level performance
