D.4.3. Executive Summary (3 page limit)
Do not include any cost information in this section of the proposal. See Section D.5.
The Executive Summary should include a clear, concise summary of the proposed
approach to the project specifications as well as the bidder’s past experience conducting
relevant projects. Additionally, a general description of the capabilities and planned roles
of any proposed subcontractor(s), a summary of the bidder’s understanding of the various
review components and required processes should be included.
Provide a narrative overview of your proposed service as transportation manager for the
New York State Medicaid Transportation Management Initiative – Finger Lakes/Northern
New York Regions. The narrative must include a “roll-out” plan that briefly describes each
phase, and include a timeline showing how the transportation management functions will be
implemented in each region. The location of the Core Management Team must be identified
and how this location will allow the management team access to the Finger Lakes Region
and/or Northern New York Region counties to fulfill the requirements of the FAS must be
explained.
