This post, recognised by the SAC in Orthodontics, builds on the initial three year
training programme and equips trainees for the ISFE in Orthodontics and provides
the preparation for Consultant appointment.
The requirements of this programme are such that the timetable included with the
Job Description is to be regarded as provisional and flexible.
Every attempt will be made to adapt the programme to the needs of the individual
trainee so that an appropriate balance of experience can be successfully achieved
over the two year training period.
THE HOSPITALS AND DEPARTMENTS INVOLVED IN TRAINING IN
ORTHODONTICS
BIRMINGHAM DENTAL HOSPITAL
Birmingham is a thriving city with a population approaching one million, and forming
part of a larger conurbation of almost 2.5million. The city has excellent road and rail
links and its airport is currently the fastest growing in the country. A host of social,
cultural and sporting activities and entertainment are readily available within easy
traveling distance. The International Convention Centre is the largest in Europe and
includes a concert hall for the City of Birmingham Symphony Orchestra. Nearby are
the National Indoor Arena, Town Hall, Museum and Art Gallery and the City Library,
itself one of the largest in Europe. There are three professional theatres in the city
and Stratford-upon-Avon is only 30 miles distance. On the outskirts of the city is the
National Exhibition Centre which, besides being the regular home of many large
national and international exhibitions, is also the venue for an increasing number of
sports and entertainment events of note.

THE DENTAL HOSPITAL
The Dental Hospital shares a purpose built facility in the centre of the City with the
University of Birmingham School of Dentistry and is adjacent to Birmingham
Children’s Hospital. The Dental Hospital provides an out patient and day case
service with approximately 130,000 patient attendances per annum, of which some
20,000 are new patient registrations.
All the dental specialties are represented within the building by consultant clinics and
University teaching Departments. Excellent liaison exists with clinical and laboratory
departments within local Hospitals and a comprehensive range of investigative and
reporting services is available to the site.
The Dental Hospital is a Clinical Directorate constituting one of the specialist
services of Birmingham Community Health Care Trust, the largest PCT in the
country.
It is proposed that the dental hospital will move to a brand new building located at
Pebble Mill in the autumn of 2015. This move will affect the successful applicant to
this post.
THE ORTHODONTIC DEPARTMENT
The Orthodontic Department is located on the Third Floor of the Dental Hospital,
adjacent to the Department of Paediatric Dentistry. The facilities are shared with the
School of Dentistry for undergraduate teaching.
There are 14 dental units, including an enclosed surgery and a large demonstration
surgery. The department has recently transferred to central sterilisation. Also within
the department is a tutorial room, a dedicated computer facility and a record model
store. The Orthodontic Laboratory is located adjacent to the Clinical area, with an
establishment of four technicians. The department has a 1wte Senior Dental Nurse
and 6.6 wte Dental Nurses plus 3.0 wte trainee Dental Nurses. There is full
secretarial support. The Orthodontic Clinical Staff establishment is:

The department runs a 3 year M.Orth/MPhil training programme, with an annual
intake of 4 Specialist Registrars.
The Orthodontic Department accepts referrals from Birmingham and the West
Midlands. In 2010 there were some 13, 000 patient attendances and approaching
1,500 new patient referrals were received.
There is a close working relationship between the Orthodontic Department and the
Oral and Maxillofacial Surgery units in Birmingham. Joint planning and review clinics
take place regularly at Birmingham Dental Hospital with Orthognathic Surgery being
carried out at the OMFS hub at University Hospital Birmingham Trust.

THE WEST MIDLANDS REGIONAL CLEFT LIP AND PALATE SERVICE
The West Midlands Regional Cleft Lip and Palate Service was the first to be
designated in England and Wales as part of the reforms following the
recommendations contained in the Clinical Standards Advisory Group’s report
(CSAG February 1998). The multi-disciplinary team works across a twin-site centre
to give a comprehensive cleft service both to children and adults – with Birmingham
Children’s Hospital acting as the paediatric hub and Queen Elizabeth Hospital acting
as the adult hub.
Orthodontic treatment services are provided at Birmingham Dental Hospital which is
closely situated to Birmingham Children’s Hospital and is the spoke service with the
major orthodontic component within the Region, as well as at other Regional
Orthodontic units.

CENTRAL OUT-PATIENTS DEPARTMENT
The area of North Staffordshire covers approximately 340 square miles and is
bordered to the west by Shropshire and to the north east by the Derbyshire Peak
District. It is separated from the Black Country to the South by some 30 miles of
agricultural land.
Stoke-on-Trent has been the centre of the pottery industry for centuries and is also a
mining area. A collection of small towns geographically very close together, it
achieved city status in 1910. Stoke-on-Trent is adjacent to the M6 (junction 15)
motorway, about one hour’s drive from both Manchester and Birmingham. There is
an Intercity rail service with London less than two hours away.
THE HOSPITAL
The University Hospital of North Staffordshire NHS Trust, serves a population of
about 500,000 residents. The Hospital provides a comprehensive range of fully
integrated acute, clinical and non-clinical services. In addition, an extensive range of
Regional Speciality services are provided which cover a population in excess of
3,000,000. In May 2012 outpatients relocated to a brand new hospital centralised to
the main site. The Department of Orthodontics and the Department of Oral and
Maxillofacial Surgery form a sub-directorate within the Surgical Directorate.
THE ORTHODONTIC DEPARTMENT
The Orthodontic Department is situated in a brand new purpose built department colocated with Oral and Maxillofacial Surgery and Restorative dentistry. There are five
orthodontic surgeries 2 restorative surgeries and multiple max fac surgeries as well
as surgical rooms and a meeting room.
A comprehensive Orthodontic diagnostic, planning and treatment service is
provided, cases for treatment within the Department being selected according to
need and complexity. Clear referral guidelines are understood by the local
practitioners and the referral protocol includes the provision of accompanying study
models. Radiographs are taken in the department by qualified dental nurses.
Contemporary clinical materials are available including access to self-ligating
brackets and use of temporary anchorage devices and fixed functional appliances.

Monthly joint clinics are held between the Orthodontic team nd Maxillofacial Surgery
and restorative dentistry .
A fully equipped dental lab is available on site staffed by 5 technicians.

IN ORTHODONTICS
POST CCST TRAINING APPOINTMENT
TRAINING PROGRAMME
The Post CCST Trainee Appointment in Orthodontics is for an additional two years
following on from the 3 year UK specialty programme. It is structured to equip
trainees to meet the regulations for the Intercollegiate Specialty Fellowship
Examination in Orthodontics (ISFE) of the Royal Colleges and to provide eligibility
for appointment to a Consultant post in Orthodontics in the National Health Service.
This post is based at a major Dental Hospital and the trainee is involved in the
service and the undergraduate and postgraduate teaching activities of a Hospital
and University Orthodontic department.
In addition, access is provided to all dental specialties with established joint working
arrangements and with an emphasis on a multi-disciplinary approach. The trainee
will be an integral member of the team providing the orthognathic component in the
joint assessment, planning and treatment of patients requiring orthognathic surgery
by the Birmingham Oral and Maxillofacial Surgical Consultants, whose in-patient hub
is centered at the Queen Elizabeth Hospital.
The trainee will be a member of the West Midlands Regional Cleft Lip and Palate
Team based at Birmingham Children’s Hospital. The orthodontic management of the
cleft patients is undertaken within the orthodontic department of the dental school.
The trainee will also attend clinics of the Supra-Regional Cranio-facial Team.

Up to four sessions each week are spent at the regional Orthodontic Unit at
University Hospital of North Staffordshire NHS Trust, Stoke-on-Trent where the
trainee is a member of the clinical team involved with the Consultants in Oral and
Maxillofacial Surgery in the management and treatment of patients requiring elective
Orthognathic Surgery.
At both centres expertise and training will be provided throughout the training period
to develop appropriate management skills. Study time is made available and further
research and publication are encouraged.
POST CCST TRAINING APPOINTMENT
1.0 AIM OF THE POST CCST TRAINING APPOINTMENT
1.1 To prepare for a Consultant post and for the ISFE in Orthodontics held by
the Royal Colleges.
2.0 OBJECTIVES OF THE POST CCST TRAINING APPOINTMENT
2.1 To provide an appropriate clinical training in the procedures of Orthodontic
diagnosis, treatment planning and case management
2.2 To gain experience in the joint management and treatment of cases
requiring elective surgery, including Orthognathic Surgery
2.3 To participate in the Team approach to the care and treatment of patients
with cleft of lip and palate
2.4 To develop expertise in the assessment and treatment of problems
requiring inter-disciplinary dental specialty management
2.5 To gain expertise in the assessment and appropriate case management of
patients whose Orthodontic treatment is to be undertaken outside of the
Hospital service
2.6 To gain experience of and be involved in Orthodontic training at
undergraduate and graduate levels, both specialist and non-specialist

2.7 To continue to develop teaching and communication skills
2.8 To gain insight into issues relevant to the provision of Orthodontic
services, to be exposed to management issues, and to develop
appropriate management skills
2.9 Through involvement in continuing professional education, clinical audit
and peer review to develop the skills of critical self appraisal in the delivery
of patient care
3.0 GENERAL CONSIDERATIONS
3.1 The period of training will follow completion of three year specialty training
and the acquisition of a Membership in Orthodontics of the Royal College
of Surgeons, or equivalent
3.2 The training programme will normally be two years’ duration
3.3 During this training period the Specialist Registrar will be expected to
publish research and observations of clinical importance

FACILITIES AVAILABLE TO THE POST CCST TRAINEE AT THE DENTAL
HOSPITAL
4.1
The Post CCST Trainee will work in a modern, fully equipped clinical
department with appropriate supporting facilities and staff including
Radiography and chairside and secretarial assistance
4.2
A dedicated Orthodontic Laboratory adjacent to the department
provides full technical support
4.3
The services of a department of Clinical Illustration and a University
department of Medical Photography are readily available
4.4
Computer facilities and digitisation are provided within the department
4.5
A well stocked Library is available within the Hospital containing a
comprehensive range of journals and text books. The Library has
connection to the High Speed Campus Network at the University and
internet facilities
4.6
Appropriate accommodation and opportunity for study is provided

5.3 The training post will be based at Birmingham Dental Hospital with
normally, four sessions each week being spent at University Hospital of
North Staffordshire NHS Trust, Stoke on Trent.

5.4 The programme will include a total of 8 clinical sessions per week, of
which 5 will be for personal treatment of patients. The detailed timetable
will be organized to allow flexibility depending on the previous specialty
training and experience of the appointee
5.5 At the Dental Hospital the Specialist Registrar will participate in joint
assessment and planning sessions with other dental specialties, including
Paediatric Dentistry, Restorative Dentistry, Oral and Maxillofacial Surgery
and Surgical Dentistry
5.6 The Specialist Registrar will be a member of the West Midlands Regional
Cleft Lip and Palate Team and will participate in the activities of that Team
including joint clinics and focused training goals.
5.7 The Specialist Registrar will have the opportunity to attend joint clinics for
patients with a mandibular deformity and provide treatment for these
patients under guidance.
5.8 The Specialist Registrar will participate in joint assessment and planning
clinics for patients requiring elective Orthognathic Surgery, and be
involved in the pre, peri and post operative care of such patients
5.9 The Specialist Registrar will be expected to be involved in the supervision
and training of junior members of staff and will participate in other training
programmes, including undergraduate teaching
5.10 There will be active participation in formal audit and quality assurance
programmes, at departmental and Regional levels
5.11 The Specialist Registrar will be expected to participate in management
and administration as part of their training
5.12 Study leave will be granted for appropriate courses, conferences and
meetings provided there is support from the Consultant in charge of the
programme and the Postgraduate Clinical Tutor
5.13 The Specialist Registrar will be accountable through the Consultant in
charge of the Orthodontic Department to the Director of Dental Services
5.14 Training progress will be assessed on a regular basis and in particular
progression in training will be monitored using the RITA process.

MAIN CONDITIONS OF SERVICE

6.1 The post is covered by National Health Service Terms and Conditions of
Service for hospital medical and dental staff (England and Wales) as
variously amended in consultation between the Trust and Staff
6.2
The post is a whole time appointment
6.3
There is no residential accommodation and no requirement to be a
resident
6.4
An appropriate Honorary contract with the University of Birmingham
will be applied for

Full registration with the General Dental Council is mandatory
6.6
The salary will be paid on Specialist Registrar scale
OTHER FACILITIES
7.1 Social and recreation facilities are available within The University of

8 CONDITIONS OF APPOINTMENT
8.1 Candidates will not be considered for appointment unless they hold a
Fellowship in Dental Surgery of one of the Royal Surgical Colleges or
Equivalent and are eligible for a CCST in Orthodontics.
8.2 CONFIDENTIALITY

8.3 INVESTORS IN PEOPLE
All employees have a responsibility to participate in regular appraisal
with their manager to identify performance standards of the post. As
part of the appraisal process, every employee is responsible for

participating in identifying their own training and development needs to
meet those performance standards
8.4 HEALTH AND SAFETY
Attention is drawn to the responsibility of all employees to take
reasonable care for the health and safety of themselves and other
people who may be affected be their actions at work
8.5 MEDICAL SCREENING
All employment with the Trust is subject to satisfactory health
screening by the Occupational Health Department
8.6 SUPERANNUATION
Any member of staff working at least half time, aged 16 or over is
entitled to join the NHS Superannuation Scheme. From 6th April 1988
members of staff are entitled to make their own personal arrangements
and are not obliged to join the NHS scheme. However, unless a Form
SD502 (available from Personnel) is completed and returned
Superannuation will automatically be deducted from the salary

Joint clinics are held at the Dental Hospital with Oral & Maxillofacial Surgery,
Restorative Dentistry, Paediatric Dentistry and Oral Surgery.
Regular attendance at the Regional Cleft Lip and Palate Centre is structured around
specific learning outcomes.
Attendance at other clinics for example, the Craniofacial Team will be organised as
appropriate.
Throughout the programme, sessions will be built-in to involve both undergraduate
and postgraduate clinical and formal teaching.
When the dental hospital relocates to the Pebble Mill site it is the intention of the
service to conduct evening clinics on two nights a week. The successful appointee to

this post may be asked to work on one or two evening sessions per week and
should be prepared to meet this requirement. The total weekly sessions worked will
remain at 10.
