Horizon Blue Cross and Blue Shield of New Jersey Title: Contrast Media for Diagnostic Imaging Section: Owner: Reimbursement Radiology/Specialty Contracting Effective Date: Revision Dates: 9/7/2009 8/2/2010 Distribution: Supersedes: Next Review Date: Policy # 047 Purpose: This publication serves as clarification to the policy presented on March 25, 2009. This policy provides guidelines for the reimbursement of contrast agents used with X-rays (including but not limited to fluoroscopy and interventional procedures), CT, CTA, CCTA, MRI, MRA and Ultrasound/Echocardiography when billed by participating and nonparticipating providers. Products: This policy is applicable to both Insured and ASO accounts for all products, with the exception of the following: • • • • • • COB, when Horizon BCBSNJ is secondary (which includes secondary to Traditional Medicare claims). Federal Employee Program (FEP). ITS Home. ITS Host Medicare Advantage. Horizon NJ Health. Medigap. Policy: This policy describes Horizon BCBSNJ’s reimbursement policy for contrast materials for both office and facility (Hospital, Ambulatory Surgery Center and Skilled Nursing Facility) settings. This policy is applicable to the codes listed below and applies to the code globally. Horizon BCBSNJ will not provide reimbursement to a physician or other health care professional for contrast materials submitted with HCPCS codes with an office or facility place of service. All codes are subject to quarterly and yearly updates as dictated by the HCPCS and AMA CPT reference source. Attachment A: Imaging CPT Code List Attachment B: Contrast Code List Limitations and Exclusions: While reimbursement is considered, payment determination is subject to, but not limited to: • • • • Group benefit. Provider contract. Routine claim editing logic, including but not limited to incidental or mutually exclusive logic and medical necessity. Mandated or legislative required criteria will always supersede. Procedure: Deny claim lines containing contrast materials listed in Attachment B, submitted with an office or facility place of service. 1 Attachment A – Imaging CPT Code List: Code Code Description
