Unit Dose Site Parameters
Overview
Introduction
Overview: The Inpatient Pharmacy Supervisor Menu is available so you can customize your medication
filling process at the user and ward levels. This menu also includes other related unit dose
parameters. Within these options, you can designate how a pharmacist finishes inpatient medication
orders. Ward setups allow you to specify such parameters as administration times, automatic stop
date/times, default printers for printing pre-exchange reports and cart exchange schedules. This menu
contains the setup option for Ward Groups, where ward locations may be grouped together, if their
medication carts are filled and/or exchanged on the same schedule. Ward groups are the basis for the
setup of pick lists used to fill the unit dose medication carts, with parameters such as sorting the fills by
room-bed or by patient name. Inpatient automation devices may be designated. You can also establish
the settings for automatic discontinuation of medication orders (upon ward transfers, service changes,
etc.). Additionally, a variety of management reports are available.

inpatient orders for outpatients. This will be a clinic already
established by your facility.
Number of days to stop
Type a number between 1 and 365, 0 Decimal Digits. This will be the
number of days the medication orders will be active in Inpatient
Medications for outpatients. (IMO)
AUTO-DC IMO ORDERS
Enter YES to allow IMO orders to follow the standard patient
movement definition. Enter NO to allow IMO orders to remain active.
SEND TO BCMA?:
Enter YES to make these orders available to BCMA, NO to prevent
these orders from being sent to BCMA.
IMO DC/EXPIRED DAY
LIMIT:
This field contains the number of days that DC/Expired orders will be
included in the enhanced order checks for drug interactions and
therapeutic duplications for clinic orders. If this field is left blank, a
default value of 30 days will be used.
For Inpatient Medications for Outpatients to work correctly, you
need to set up the clinic definitions and have access to this
scheduling package option.

This will produce a report to show the dispensing cost of the
pharmacy by ward. Only those wards with a dispensing amount or
cost are shown. The dispense drug and quantities dispensed are
included in the report.

This report calculates the total cost for all or selected drugs
dispensed over a specific time frame of at least one calendar day. The
information can be sorted for optimization.

This 80 column report lists patients on specific primary drug(s),
dispense drug(s), or VA class(es) of drugs. If more than one primary
drug, dispense drug, or VA class of drug is chosen, only patients with
orders containing all the drugs or classes will be shown. The user may
choose to list all orders, only IV orders, or only Unit Dose orders.
Provider (Cost per)

This prints the cost of all drugs dispensed within a user-specified
length of time, grouped together by provider.
Service (total cost per)
[PSJU SCT]
Creates a report of the total cost of dispensed medications for ward
services (medicine, surgery, etc.) over a user-specified time frame in
terms units dispensed and average cost.
Total Cost to Date
(Current Patients)
[PSJU TCTD]
This will be the total cost of unit dose medication dispensed for
patients that are currently admitted at the medical center.
RECOMMENDED ACTION

This option allows the user to print a listing of all the active orders
(during the timeframe selected) that contain problem schedules.
These are either invalid schedules or schedules with abbreviations
that Joint Commission considers dangerous.

Search
This option allows the user to process Inpatient Medications order for
the specified time period and identify all schedules in use. In addition,
the option will identify all schedules that are not currently in the
ADMINISTRATION SCHEDULE file (#51.1). The option will send a
mailman message to the person who initiated the routine.
Order Set Enter/Edit [PSJU OSE]
Order Set Enter/Edit
Allows the user to create and/or edit ORDER SETS for use in order
entry. An ORDER SET is one or more orders grouped together to be
entering for a patient at one time, by entered a "S." and the name of
the order set at the DRUG prompt in order entry.
As orders are entered directly into CPRS
by the providers, this option is rarely
used. However, it may be helpful to set
up order sets to use for entering orders
for training/test patients. If the order
set is set up on the production account
it would be copied with any “mirror”
accounts for training.

ORDER ACTION ON PATIENT TRANSFER
The Inpatient Medications package has changed the criteria for the
auto discontinuing of a patient's IV & Unit Dose orders whenever the
patient is transferred between wards and/or services. The decision to
d/c Inpatient orders is now determined by the site on a ward by ward
(or service by service) basis. While this new process will entail initial
extra set up on the site's part, it will allow the site almost complete
control of the auto discontinue process. The set up for this process
involves three main steps:
1. If your site wishes to have Inpatient orders d/c'd on all or most
ward transfers, you can have the package automatically set up all

made on a ward-by-ward and/or

2. Ward transfers - Select a 'from' ward. This is a ward from which a
patient may be transferred. For each 'from' ward, you can:
A. Select an 'On Pass' action. This is the action the Inpatient
Medications package will take on a patient's orders whenever the
patient is transferred
from the selected 'from' ward to authorized absence less than 96
hours (known as 'on pass'). The actions include:
1. discontinue the orders
2. place the orders on hold
3. take no action
B. Select an 'Authorized Absence' action. This is the action the
Inpatient Medications package will take on a patient's orders
whenever the patient is transferred from the selected 'from' ward to
authorized absence greater than 96 hours. The actions include:
1. discontinue the orders
The same functionality is available in
CPRS parameters. It is each site decision
whether the setup is in CPRS or
Pharmacy Auto-discontinue setup.
This is a multidisciplinary function, and
should be coordinated with your site’s
Clinical Application Coordinator.
For authorized absence, i.e., “on pass”
off of a ward, the action set up here
determines whether the orders are
discontinued, placed on hold or no
action at all. Note if the orders are to
be discontinued, it will impact the units
to be dispensed on the pick list and
updates.

C. Select the 'To' wards. Whenever a patient is transferred from the
selected 'from' ward to any of the selected 'to' wards, the patient's IV
and Unit Dose orders will be d/c'd. For example, if '1 North' is
selected as a 'from' ward and '1 South' is selected as a 'to' ward, any
time a patient is transferred
FROM 1 North TO 1 South, the patient's Inpatient orders
will be discontinued.
This process is 'one way' only. For example, if your site also wants
orders to be d/c'd whenever a patient is transferred FROM 1 South
TO 1 North, you
will have to enter 1 South as a 'from' ward and then enter 1 North as
one of its 'to' wards. An example for when this would be useful might
be to have
orders d/c'd when patients are transferred from Medicine wards to
Surgery wards, but not d/c'd when patients are transferred from
Surgery wards to Medicine wards.
3. Service transfers - Select a 'from' service. This is a service from
which a patient may be transferred. For each 'from' service, you can:
A. Select the 'To' services. Whenever a patient is transferred from
the selected 'from' service to any of the selected 'to' services, the
patient's IV and Unit Dose orders will be d/c'd. For example, if
'Medicine' is selected as a 'from' service and 'Surgery' is selected as a
'to' service, anytime a patient is transferred FROM Medicine TO
Surgery, the patient's Inpatient orders will be discontinued.
This process is also 'one way' only. For example, if your site also
wants orders to be d/c'd whenever a patient is transferred FROM
Surgery TO Medicine, you will have to enter Surgery as a 'from'
service and then enter Medicine as one of its 'to' services. If all of
your wards are set for auto d/c, it is not necessary to enter services.
If there is a specific ward or service for which your site does not want
Inpatient orders d/c'd, you need only delete the 'to' ward or service.
Inpatient orders are always automatically d/c'd whenever the patient

ORDER ENTRY
PROCESS:
HELP-PROMPT: This is the type of order entry process to be used by
this user. The REGULAR order entry is the full set of prompts for the
entry of an order, after which the user is shown a full view of the
order and allowed to take immediate action on the order. The
ABBREVIATED order entry gives the user fewer prompts for the entry
of an order, after which the user is shown a full view of the order and
is allowed to take immediate action on the order. The WARD order
entry gives the user the same prompts as the ABBREVIATED order
entry, but then gives a brief view of the entered order and does not
allow immediate action to be taken on the order. No entry here is
the same as selecting REGULAR order entry.
This parameter only affects the entry of
orders using the VISTA Unit Dose Order
Entry or Inpatient Order Entry, in
situations where the order was not
entered in CPRS, for example, during a
period of extended downtime, where
orders may be been written (on paper).

HELP-PROMPT: Allows user to edit the Inpatient Ward Parameters.
These parameters determine how the Inpatient Medications package
will react for users acting on patients who reside on the
corresponding wards.

HELP-PROMPT: This is the number of days a standard order should
last. The first order entered for a patient uses this number to
calculate a default value for the order's STOP DATE/TIME. This
number is also used if SAME STOP DATE has no entry, or an entry of
"NO".
The “auto-stop” date & time may be set
individually for each ward location
based on your site policies. For
example, Medical / Surgical wards may
be 28 or 30 days or Extended care units
may be 84 or 90 days.
The maximum is 100 days.

ONE-TIME:
HELP-PROMPT: Enter the number of days a one-time order should
last. The number can be from 1-100, however, it cannot exceed the
number of days that standard orders last (DAYS UNTIL STOP

on the profile. If the one-time order
needs to be available for more than the
auto-stop, the pharmacist can change
the stop date when finishing the order.
The order will fall off from BCMA once
it is administered, or at the auto-stop if
