i ABSTRACT BACKGROUND: Drug therapy problems (DTP) pose significant challenges to health care providers and affect morbidity, mortality and a patients quality of life. There is a high prevalence of drug therapy problems in a population with diabetes and hypertension. OBJECTIVE: To identify the drug therapy problems that occur in prescriptions of the diabetic and hypertensive patients; to determine the frequency of their occurrence and the causes of drug therapy problems and to relate the DTP with the demographics of the patients. METHOD: This descriptive retrospective study was conducted at a tertiary institution using patients medical files that have been diagnosed of both diabetes and hypertension. Their prescription over a one year period were assessed; data was collected using a designed data collection form. Data was analyzed using ANOVA. RESULTS: 92 patients folders with a total of 619 prescriptions were assessed and 407 DTPs Identified, the most common DTP encountered was inappropriate adherence (IA) (43%) followed by drug interaction (DI) (14%) and wrong drug (WD) (12%) and dose too high (DTH) (11%). The major cause of the problem being that patients prefers not to take their medications. There were more females 54.3% than males, more urban dwellers (59.72%) than rural dwellers (40.22%). Mostly the trading occupation has the highest no of DTP. The mean age of the subjects was 60-69 age range and they had the highest frequency of the DTP alongside the 50-59 age groups .The drug therapy problems are independent of patients social demographic. CONCLUSION: Early identification of the types and the causes can enhance the prevention and management of DTPs in type 2 diabetes mellitus (T2DM) with hypertension. Pharmacists included in the health come team can improve quality of life of patients with diabetes mellitus and hypertension through pharmaceutical care practices
