BAR – bright, alert, responsive
PE - physical exam
BARH - bright, alert, responsive, hydrated
EENT - ears, eyes, nose, throat
AMB - usually the abbreviation for ambulatory
QAR - quiet, alert, responsive
ABD NSF - abdomen, no significant findings
QARH - quiet, alert, responsive, hydrated
(generally referring to abdominal palpation)
H&L - heart and lungs, Sometimes abbreviated
NSF - no significant findings

NOSF - no other significant findings
BCS - body condition score
WNL - within normal limits
INT - integument (skin)
DZ - disease
KCS - keratoconjunctivitis sica, also known as
dry eye
HX - history
NMA - no murmurs or arrhythmias
TX - treatment
GI - gastrointestinal
SX - surgery
GU - genitourinary
BX - biopsy
MS or MSK - musculoskeletal
RX - prescription/prescribe
LN - lymph nodes
FX - fracture
ENDO - endocrine
CWCT - continue with current treatment
POS - positive

NEG - negative
ND - nasal discharge
OD - ocular discharge
TPR - temperature, pulse, respiration
SOD, SND - serious ocular discharge, serous
HR - heart rate

RR - respiratory rate
MPND - mucopurulent nasal discharge
DTM - determatophyte test medium, a culture
MM - mucus membranes
for ringworm
CRT - capillary refill time, a measurement of
perfusion. Normal should be < 2 seconds
CP - conscious proprioception
SNP - soft, non-painful (usually referring to
abdominal palpation)
F/S - female spayed
FIP - feline infectious peritonitits
M/N - male neutered (sometimes indicated M/C
for male castrated)
CBC - complete blood count
CHEM - chemistry panel
IV - intravenous
U/A - urinalysis
IM - intramuscular
SQ - subcutaneous (sometimes indicated SC)
CYSTO - cystocentesis; collection of urine
IP - intraperitoneal
