Florida Virtual School (FLVS) Enrollment Policy August 26, 2015 Dear Maclay Upper School Students and Parents, The policy for enrolling in Florida Virtual School classes is outlined below. Both students and parents must agree to these terms and sign this form below. Please contact the appropriate administrator for your class if you have any questions. Very Sincerely, Charles Beamer Head of Upper School 1. Summer Enrollment for courses taught at Maclay (e.g. Economics, Algebra II, Psychology): a. Students may enroll in an approved FLVS course for the summer as long as it is completed BEFORE the first day of Maclay Fall term classes (17 August for 2015). b. Students should avoid trying to complete year-long (2 semesters/segments) courses as the deadline remains the same. c. Students must communicate to the FLVS instructor that they must be on an accelerated program in order to complete the course by OUR deadline, which is much faster than the typical 18-week semester timeline for the FLVS. d. IF students do not complete the FLVS course by the first day of school, they will not receive credit for that course and must complete the course at Maclay. e. Students must register online at www.FLVS.net for summer school classes by APRIL 15th. During registration, use Maclay School as the school name. We have no District code or student ID. 2. Enrollment for academic courses NOT taught at Maclay (e.g. Geography, Global Studies, etc.): a. Students may enroll in an approved FLVS course that Maclay does not offer at any time during the school year or summer and follow the typical FLVS timeline. Such courses must be the 6th academic course on the student’s curriculum. Thus, all students must first enroll in the minimum 5 ACADEMIC courses from the Maclay curriculum in addition to an FLVS course. b. Students must go to the LIBRARY during their FLVS period each day. c. Be aware that there are significant time management challenges when trying to complete a full schedule at Maclay while also working on a FLVS course. 3. Students must communicate with their FLVS instructor if they encounter any unforeseen delays in their studies due to illness, travel etc. If the student fails or withdrawals from a course, the student must notify the appropriate Maclay School Administrator. 4. ALL successfully completed FLVS courses within these guidelines will be annotated on the Maclay School Transcript. 5. Any exceptions or changes to this policy must be approved by the Head of Upper School. 6. Students and parents must sign below, certifying that they agree to the terms of this policy and turn-in the form to the appropriate Maclay School Administrator (Seniors-Dr. Hampton; Juniors-Mr. Beamer; Sophomores-Mr. Andrews/Mrs. Bakas & Freshman-Mrs. Croston). ________________________________________________________ ________________________________________________________ Student: Printed Name/Signature Parent Printed Name/Signature __________________________________ Administrator Signature __________________________________ FLVS COURSE NAME
