Treatment principle: “Dentoalveolar compensation or orthodontic surgery”

Dentoalveolar compensation
 Extrusion of anterior region, retroclination of anterior region, intrusion and/or grinding-in of
posterior region in class I and II, planned orthodontic extractions, prosthetic measures
 Indication is skeletal*** for skeletal open bite depending on degree and dentoalveolar for
anterior open bite depending on reaction
Orthodontic surgery – orthognathic surgery
 Dental correction: pre- and post-surgical orthodontics, no extrusion, if necessary even
intrusion of the anterior region: “orthodontic relapse should not be added to surgical relapse”
 Skeletal correction: different osteotomies or distractions (cranial movement of the maxilla:
Le Fort I with anterior mandibular rotation, bimaxillary osteotomy, Delaire-Joos osteotomy)
 Indication: extreme dysgnathia, contraindication to dentoalveolar compensation, e.g.
cooperation, function, aesthetics, stability, periodontal risks.
