ICD-9 Codes
To avoid unwarranted denials, requisitions must include all medically appropriate ICD-9 diagnosis codes. ICD-9 diagnosis codes are required by all third party payors to confirm the medical necessity
of the test(s) and/or profile(s) ordered. The codes listed below are some of the most commonly received codes and were taken from the 2004 ICD-9 manual issued by the AMA. While this list may
be a useful tool, it is not complete. Please refer to the ICD-9 manual for a complete listing. The ultimate responsibility for correct coding lies with the ordering practitioner. Please place all medically
appropriate ICD-9 codes on the requisition in their order of importance. If ordering testing solely for screening purposes, please provide the appropriate screening ICD-9 code(s).
Providers are encouraged to code the requisition that describes the patient’s condition using terminology that includes specific diagnosis as well as symptoms, problems or reasons for the encounter.
If symptoms are present, but a definitive diagnosis has not yet been determined, code the symptoms.
