Normal thymus, 3-month-old male. A: Chest radiograph—there is widening of the superior mediastinum with a wavy left border typical of a thymic wave
sign, present where the thymus is indented anteriorly by the ribs. B: Axial contrasted computerized tomography (CT)—the quadrilateral shape of the infant
thymus and indentation of the left side of the thymus by a left anterior rib is demonstrated (arrow). C: Coronal reconstruction demonstrates the superior
and inferior extent of the thymus, extending all the way down to the diaphragm adjacent to the right atrium (arrow). In addition, there is a small superior
extension of the thymus into the left neck anteriorly (arrowhead); this is also a common normal variant.
