YOU AND YOUR MEDICAL DATA
There have been three new data schemes introduced recently to extract
information about a patient from the GP medical record. These schemes are the
‘Summary Care Record’, ‘Connecting Care’ and ‘Care.data’. Patients are
automatically included in these schemes unless they express a wish to be
excluded.
The Summary Care Record (SCR) takes less information about you from our
records and is designed to be a basic reference for staff in an emergency care
setting. This means that if you attend an A & E department anywhere in the
country they will know immediately about your current medications, allergies from
which you suffer and any bad reactions to medicines in the past. This will help to
treat you safely.
Connecting Care is a local initiative covering South Gloucestershire, Bristol and
North Somerset. A summary of information about a patient will be taken from
various care organisations, and this creates one combined record providing a
complete picture about a patient to healthcare professionals. This means that
should you require emergency treatment in a local setting, the clinician treating
you will know about your existing health problems (excluding any sensitive
information), your current medication, any allergies you have as well as support
or care packages you have available to you in your home. Likewise your GP will
have access to information about any hospital treatment you have received, any
diagnostic tests that were undertaken outside the surgery, any additional
medication prescribed by a consultant or hospital doctor, and whether you are
under the care of community services like the District Nurses.
This record will only be used to support your care locally. It will ensure that the
people caring for you have enough information to treat you safely and to coordinate your care.
Care.data this has been commissioned nationally by NHS England and will be
delivered by the Health and Social Care Information Centre (HSCIC) to help plan
services and the provision of healthcare for the nation in the future. Most of the
extracted data will be used anonymously, that is, all indentifying information like
names, dates of birth, addresses and NHS numbers will be concealed. However
under the powers of the Health and Social Care Act 2012, the Health and Social
Care Information Centre (HSCIC) can, under certain circumstances, require
Personal Confidential Data (PCD) from GP practices.

records used by the Health and Social Information Centre (HSCIC) complete
the form (page 2) stating that you wish to opt out and return to the practice – we
will input the relevant read code onto your record to prevent your information
being shared.

What does it mean if I DO NOT have a Connecting Care local record?
Health and Social Care staff caring for you may not be aware of your current medications,
allergies you suffer from and any bad reactions to medicines you have had, in order to treat
you safely in an emergency.
Your records will stay as they are now with information being shared by letter, email, fax or
phone.
Your request to opt out of Connecting Care will be processed within seven working days of
receipt of the completed form and you will receive a letter to acknowledge that your record
has been updated. If you do not receive confirmation within seven working days, please
contact PALS, on the telephone number below.
