Formative Assessment Grade Level: 8th Subject: Mathematics Prepared By: Bill Sebring State Standard (# and verbiage): (8.1.D): Determine the slope and y-intercept of a linear function described by a symbolic expression, table, and a graph. Kid Friendly Objective I will be able to state the slope and y-intercept of a linear relationship if I am given a table, a graph, or a symbolic expression. Formative Assessment Strategy Exit Slips Three representations of linear relationships will be given on the board (table, graph, symbolic expression). Next to each will be the slope and y-intercept for the given representation. Each will show a common mistake such as confusing the y-intercept and the slope, interchanging the difference in y and the difference in x, and not finding the correct y-intercept from the table. Upcoming opportunities to use this formative assessment: Problem 2.2 Materials Needed: Three representations of linear relationships and work showing the finding of the slope and the y-intercept. Additional Notes Information I can get from this formative assessment: Can students find the slope from a graph? Do students know where to find the slope and the y-intercept in y= mx + b? Can students find the slope by finding the change in y over the change in x?
