Doctoral Thesis Estudo sobre o potencial imunogênico da dentina: contribuição para a etiopatogenia da reabsorção dentária Hidalgo, Mirian Marubayashi Consolaro, Alberto http://www.teses.usp.br/teses/disponiveis/25/25136/tde-21032005-084631/ English/Portuguese versions Comments of abspathology Antonio Schütz The thesis defended by authors is not recent because would have yet discussed and defended by Gregory Kind and Frank Courts, cap 18 of the book, “The biology of tooth movement,” Louis Norton e Charles J Burstone, CRC, Boca Raton, Florida, 1989. Besides, in 1997, in the thesis of doctorate defended by Professor Lara at FOB-USP, there was also an attempt of the correlation of the initial process inflammatory induced by fragments of demineralized dentin from intact impacted tooth – which in my opinion evolutes to bone and/or dental formation - with dental resorption process; whose hypothesis was published at J. dent. Research (http://jdr.iadrjournals.org/cgi/content/full/82/6/460): “Our results are consistent with the hypothesis that dentin can induce an inflammatory response and may be involved in the pathogenesis of inflammatory root resorption, in concert with other conditions such as trauma and/or bacterial products”. Is difficult to understand the methodology supporting that creative hypothesis. I do not know to that type of dentin Professor Lara refers herself. Certainly is not from an intact tooth. For that, the actual results indicate to induce bone and/or dental regeneration. As would be possible to extrapolate data obtained from coronal dentin of an intact tooth to root dentin of teeth with root resorption? The logical think suggests that the process inflammatory was caused by proteinaceous contamination of dentin such as reported in the group two of the study of Machado ME, Souza AS, Araujo VC (2004) in that root dentin with proteinaceous contamination only induced a inflammatory process; or genetically altered dentin, in the case of teeth in idiopatic resorption. In addition, denaturation of proteins of organic dentin matrix, induced by EDTA, at the least in part, might also explicate so as the inflammatory process identified by Professor Lara without any relation with resorption process hypothesized as its low antigenicity also without any relation with resorption process, but with dental and/or bone formation. Considering that Professor Lara did not mention to have studied teeth in resorption, the former hypothesis is the most probable. Believe that if the study go repeated with intact teeth non-impacted and submitted to statistical analyzes would have been too identified the presence of some proinflammatory cytokines and growth factors without significant statistical difference as well as without any relation with resorptive process, but indicating the inflammatory initial phase that evolutes to the dental/or bone regeneration such as the incipient study of Catanzaro-Guimarães SA, Catanzaro Guimarães BP, Garcia RB, Alle N (1986) indicated. Therefore, Hidalgo and Consolara thesis is only an attempt of justifying an equivocate conclusion of the initial inflammatory phase of dental and/or bone repair process induced by demineralized dentin matrix as inducing resorptive process. For the contrary, the low antigenicity resultant of protein denaturation promoted by EDTA may be necessary to orientation of the mineralization process. Well! But that is a detail of this valorized thesis, defended with force and enthusiasm, which does not compromise its honor and distinction, particularly, for whom has colleague in the examiner and editorial boards. For others, details much less inaccurate may result in a 9.999999 only with distinction. Particularly, think that this type of “thing” is used to manipulate and/or to fraud the public concourses of ingress and progression in the academic career at the Brazilian universities, favoring parents and friends of Brazilians Professors and/or of persons who detain the administrative public, political and military power. Similar process of manipulation or fraud occurs in relation to the publication of the Brazilian academic production in that the Brazilian Professor who possesses access and financial resources to international periodic elaborates the article, simply introducing the name of the parent or friend as coauthor in scientific articles or in academic books, the example of the “10 with honor and distinction”. In spite of agreeing with the possible immune theory for the dental resorption, I disagree that it is caused by kidnapped antigens released from dentin such as was affirmed for the authors; but so only that the process of resorption - in pathologic states - is caused for the incorporation of LPS to dentin structure (fragments); as well as induced for cytokines, factors of growth and other resorptives factors secreted in the local inflammation promoted by traumas, dental movement and other causes, inasmuch as the kidnapped antigens in normal dentine - in its great majority - presenting neo-formation properties, such as for the increase of the bone-inducing activity of rhBMP-2 by DPP as of a co-factor of rhBMP-2 in vivo (Saito T, Kobayashi F, Fujii T, Bessho K., 2004), as well as growth factors to dentin. Is very strange that Hidalgo and Consolara (2001) had concluded that the extracted of dentin used may induce via immune the bone or dental resorption because these two proteins (DPP and BMP) – according to its analysis - were present. With basis in the in the initial studies of Catanzaro-Guimaraes SA, Catanzaro Guimaraes BP Garcia RB, Alle N, 1986 as well as in the dissertation work of Hidalgo and Consolara ( http://www.teses.usp.br/teses/disponiveis/25/25136/tde-21032005-084631/ ), who researching the process of dental reabsorption concluded that that process would be caused for the liberation of sequestered antigen inside organic matrix of dentin, defended at the Bauru School of Dentistry at the Sao Paulo University (FOB-USP), I strongly suggest that dentin matrix of intact teeth may be used in the bone regenerative therapy as a scaffold bone, conjunctly with stem cells, such as undifferentiated cells of dental pulp, because among other mechanisms, demineralization promotes the liberation of proteins sequestered in dentin, the example of DMP-1 and LMP-1, which are able to induce to differentiation from stem cells into osteoblasts. Due to the structural, mineral and mechanic similarities between dentin and bone that type of natural scaffold bone may possess advantage in relation to others contend Has; considering that the process of mineralization of this type of scaffold bone may be easily modified to improve mechanic proprieties as stiffness, toughness and bending strength. The fact of the level of dentin protein may be used as a biological marker of the dental resorption during the dental movement (Balducci L, Ramachandran A, Hao J, Narayanan K, Evans C, George A., 2006) is not an indicative that these proteins have potential to induce dental or bone resorption via immune, but so only that were removed of the dental root by action of an active force, inducing at the bone level, neo-formation as well as cementogenesis. Evidently that these proteins when isolated from the extract and individually tested, by dependent mechanisms of the concentration, but without any relation with their antigencity may induce dental resorption by immune mechanism associated with LPS and, predominantly, non-immune via RANKr. As is difficult the identification of DPP in dentin extract, is possible that the activity resorptive to be explicated for the decreased fraction of DPP in extract. Might have also occurred error in the interpretation of fractionating since the resolution of the band was not ideal, having been interpreted MMP-9 or one of their products as DPP, such as the activated form of TGF b, considering that this protease migrates in similar position, being also involved in the activation of that growth factor as well as to the initial processes of formation of dentin and bone. Considering that the fractioning in HPLC turns evident, predominantly, fractions between 44 and 25 kDa, with tops in approximately 36 and 29 KDa (fig. 10) or near to fraction of 40 KDa in that was not identified statistic difference to the control group with relation to antigencity, is very difficulty to accept the theory that dentin protein from an intact tooth is able to cause resorption by activation of the immune response since fractions with similar molecular mass (32 and 24 KDa) have been associated to low antigenicity and with the process of the nucleation of hydroxyapatite (Domenicucci C, Goldberg HA, Sodek J. 1997). With HPLC was also identified a top in 40 KDa, indicative of the presence of osteonectin, also involved in the processes of formation and mineralization of dentin. Besides, the decreased fraction of KDa < 25 might indicate the presence of activators molecules of osteoclasts or odontoclasts need to liberation of TGFb and other growth factors of the extra cellular matrix involved in the dentinogenesis and osteogenesis. Possibly this process, which in some pathologic states, is exacerbated for the presence of LPS was bad interpreted for the authors as caused for an immune response to antigens sequestered in dentin during the odontogenesis; considering that data obtained with HPLC (fig. 10) indicate the presence of a little fraction with molecular mass superior to 100 KDa, with accentuated antigenicity. Is important to salient that the simple identification of antigenic molecules by SDS-PAGE does not permit to infer that would have induced or that will induce a immune reaction, because it depends of its concentration in the fraction, which HPLC indicated to be low; confirmed for the low production of anti-bodies in the serum of rabbit immunized with dentin extract in dust. Moreover, considering the accentuated differences existents between the biochemistry composition of the coronal and root dentin is very difficult the extrapolation of results obtained from coronal dentin to resorption in root dentin. Well! Without wanting to be repetitive, these data are details that do not remove the merit of this valorized thesis defended with force and exacerbated enthusiasm as well as laureates with 10 more louver and distinction. But in spite of the conclusion of Hidalgo and Consolara, such as in the thesis of Professor Lara, to be equivocate, to the contrary of the latter, in the former, if for a side the results contract the defended thesis Hidalgo and Consolara, confirming the low destructive antigenicity of the demineralized dentin matrix, for other, they confirm the thesis for us defended that demineralized dentin extract for its neo-formation proprieties and low antigenic may used in the periodontal regenerative therapy actuating as a bone scaffold possibly showing advantage in relation to others material contending HAs. Yet in the case of Lara’s thesis is difficult to find some rational utilization for them. With relation to the corporative and fraudulent academic system of evaluations of academic thesis that objective of turning more easy the ingress of parents and friends of the detectors of academic, administrative, politic and military Brazilian power in the academic carrier, in the defense of my thesis did not obtain more than 9.9999. http://www.teses.usp.br/teses/disponiveis/25/25136/tde-30052006-163226/. Therefore, the theory for me defended is that the dental resorption occur predominantly for non-immune mechanism dependent of the concentration of growth factors normally expressed in dentin, via increase of the expression and activation RANKr on resorptive cells, with consequent generation of cytokines, growth factors and other resorptive factors; as well as via immune mechanism dependent of the concentration of dentin protein associated to LPS removed of the tooth root during the dental movement, trauma or other factors. In the case of the “idiopathic” dental resorption, it may caused by genetic alteration in the expression of the growth factors normally expressed in the dentin altering their concentration and activating the resorptive cells via activation of RANKr. Conclusions 1. Dentin extracts of demineralized normal dentin show neo-formation proprieties of bone or cement and may been utilize in the regenerative periodontal therapy; 2. They induce a initial resorption process need to liberation of citokynes and growth factors of the extra cellular matrix, which alter their concentration so much in bone and dentin with evolution to the repair; 3. In pathologic conditions, the resorptive activity of few dentin proteins is caused by their concentration in the local of resorption, without any relation with their greater or lower antigenicity; 4. In these conditions, in the dependency of the concentration, some few dentin proteins, when individually tested, can present resorptive activity via non-immune mechanism; 5. In the dependency of concentration, these dentin proteins, when individually tested, and associated with LPS may present resorptive activity via immune mechanism; 6. Dental Resorption occur predominantly by non-immune mechanism dependent of the concentration, via augment of the expression and activation of the receptors RANKr in resorptive cells; 7. Dental resorption dental observed during the dental movement or when induced by traumatism is basically caused for the activation of non-immune mechanisms induced for the active force; 8. Idiopathic dental resorption possibly is caused by an genetic alteration that alter the normal concentration of growth factors to dentin, or their receptors, inducing the dental resorption via augment of the expression and activation of RANKr in the resorptive cells, as well as inducing the differentiation of their progenitors cells. 9. Além do que, em 1997, houve uma tentativa da professora Lara, de correlacionar a fase inicial do processo inflamatório induzido por fragmentos de dentina desmineralizada de dentes hígidos, associado ao processo de reparo, com o processo reabsortivo, cujos resultados foram publicados no J. dent. Research (2003) (http://jdr.iadrjournals.org/cgi/content/full/82/6/460). É, tão-somente, uma tentativa de justificar a conclusão equivocada da fase inflamatória inicial do processo de reparo como associado ao processo reabsortivo. Mas isso são detalhes que não tiram a distinção e o louvor dessa valorizadas teses defendidas com força e entusiasmo pelos autores, particularmente para quem tem colegas nas bancas examinadoras e corpo editorial. A despeito de concordar com a possível participação de mecanismos imunes na etiopatogênese de algumas das reabsorções dentárias, discordo de que sejam causadas pela liberação de antígenos seqüestrados na dentina, tal como afirmaram os autores; mas, sim, em estados patológicos, pela incorporação de LPS à estrutura (fragmentos) dentinária exposta, bem como induzida por citoquinas, fatores de crescimento e por outros fatores reabsortivos, sintetizados e secretados no local de inflamação, decorrente de traumatismos, movimentação dentária e de outras causas; porquanto os antígenos seqüestrados em dentina hígida – na sua grande maioria -, apresentam propriedades neoformadoras dental ou óssea, tal como ocorre pelo aumento da atividade indutora óssea da BMP2, induzida pela DPP (Saito T, Kobayashi F, Fujii T, Bessho K., 2004), também, agindo como fatores de crescimento para a dentina. Também entendemos que a dentina desmineralizada pode agir como um “scaffold” ósseo apresentando vantagens em relação aos outros biomateriais contendo HÁS naturais. Estranho que Hidalgo e Consolara (2001) tenham concluído que o extrato de dentina hígida, utilizado em sua tese de doutoramento, apresente potencial imunogênico para induzir reabsorção dentária, considerando que, segundo a sua análise, essas duas proteínas estavam presentes (DPP e BMP). O fato do nível de algumas das proteínas, presentes no extrato dentinário, poder ser utilizado como um marcador biológico da reabsorção dental, durante a movimentação dentária ortodôntica (Balducci L, Ramachandran A, Hao J, Narayanan K, Evans C, George A., 2006), não é um sinal de que são indutoras dessa reabsorção; mas, tão-somente, que foram removidas da superfície radicular, por ação de uma força ativa, sendo hábeis à indução de neoformação óssea e à cementogênese. Evidentemente, que, tais proteínas, quando isoladas do extrato dentinário e testadas isoladamente, podem, por mecanismo dependente da concentração, mas, sem qualquer relação com sua maior ou menor antigenicidade, induzir à reabsorção dental por mecanismos imunes - quando associados a LPS -, e, predominantemente, não-imunes, via RANKr/RANKL. Contudo, ressalto, que o extrato dentinário desmineralizado de dentes hígidos, tal como o utilizado na tese de doutoramento de Hidalgo e Consolara (2001), apresenta atividade neoformadora. Como é difícil a identificação de DPP - em extratos de dentina -, é possível que, ao contrário do afirmado, o extrato utilizado, ser composto de uma fração reduzida de DPP, ou ter tido a interpretação errônea da presença de MMP-9, ou de um de seus produtos como DPP, tal como a forma ativada do TGFb, porquanto essa proteinase migra na mesma posição da DPP; estando também associada à ativação desse fator de crescimento, bem como ao processo inicial de formação da dentina e osso. Pode ter ocorrido erro dos autores na interpretação do fracionamento, pois a resolução dos picos - como admitiram -, não foi a ideal para a sua conclusão. Considerando que o fracionamento em HPLC evidenciou, predominante, frações de KDa entre 44 e 25, com picos em aproximadamente 36 e 29 KDa (fig. 10), próximas à concentrada fração de 40 KDa, em que não foi identificada diferença estatística para o grupo controle - quanto à antigenicidade -, é difícil aceitar a tese proposta, porquanto frações com similar massa molecular têm sido associadas à fraca antigenicidade, bem como ao processo de nucleação da hidroxiapatita (Domenicucci C, Goldberg HA, Sodek J. 1997). A propósito disso, a HPLC indicou pico em aproximadamente 40 KDa, que na dentina de ratos, indica à presença de osteonectina, também, envolvida no processo de produção e mineralização da dentina e osso, em humanos. Além do que, o baixo pico (fig. 10), indicativo de frações com massa molecular inferior a 25 KDa pode indicar a presença de uma reduzida fração de moléculas ativadoras de osteoclastos ou odontoclastos, necessária à liberação do TGFb e de outros fatores de crescimento da matriz extracelular, envolvidos no processo na formação da dentina e osso. Possivelmente, esse processo, que em alguns estados patológicos, é exacerbado pela presença de LPS, foi erroneamente interpretado pelos autores como decorrente da ativação de uma resposta imune patológica, a antígenos seqüestrados na dentina, durante o desenvolvimento; considerando que os dados obtidos pela cromatografia líquida de alta precisão ou pressão (fig. 10) indicam a presença de uma pequena fração com massa molecular superior a 100 KDa, de acentuada antigenicidade. Vale lembrar que a simples identificação de moléculas antigênicas por SDS-PAGE não permite inferir que tenham induzido ou que venham induzir uma reação imune, pois esta, também, depende da concentração dos antígenos de alta antigenicidade, que a HPLC indicou ser baixa; confirmada pela fraca produção de anticorpos, no soro de coelhos imunizado com o extrato de dentina em pó. Ademais, considerando as acentuadas diferenças existentes na composição bioquímica das dentinas coronal e radicular é difícil extrapolar os resultados obtidos com extratos de dentina coronal para reabsorções em dentina radicular, tal como tentaram provar os autores. Bem! Mas isso, são detalhes que não tiram o mérito dessa valorizada tese, defendida pelos autores com força e exacerbado entusiasmo, laureada com 10, louvor e distinção pela erudita banca examinadora. Se por um lado vêm de encontro à tese defendida pelos autores, indubitavelmente, por outro, vêm ao encontro da tese por nós defendida, de que o extrato de dentina hígida desmineralizada, por suas propriedades neoformadoras e fraca antigenicidade, pode ser utilizada na terapia periodontal regenerativa. A propósito do corporativo e fraudulento sistema de avaliações de teses acadêmicas brasileiras, que objetiva o favorecimento de amigos e parentes dos detentores do poder acadêmico, administrativo, político e militar brasileiro, em futuros concursos públicos de ingresso e progressão na carreira docente, na defesa de minha tese de doutoramento não obtive mais do que 9,9999 (http://www.teses.usp.br/teses/disponiveis/25/25136/tde30052006-163226/). Portanto, sinto-me completamente à vontade para comentar qualquer tese da Faculdade de Odontologia de Bauru da Universidade de São Paulo (FOB-USP), particularmente do Departamento de Periodontia, cujo professor dr. Sebastião Luis Aguiar Greghi, foi o único membro de minha banca examinadora a conceder-me nota inferior a 10. Se bem que empregando a mediana, foi também 10 com distinção. Só faltou o louvor, talvez por não tomar parte do porco e corporativo meio acadêmico brasileiro (não sou professor), e assim não dificultar, ou justificar a manipulação ou fraude de algum daqueles concursos, em favor de algum dos indicados da porca mamata do meio acadêmico brasileiro. Mas, continuando o comentário a respeito dessa valorizada tese da FOB/USP, orientada pelo “mago” da atual odontologia brasileira, em minha especialidade, o perfil imunogênico induzido pelo extrato de dentina hígida desmineralizada - se não contaminada por LPS -, é compatível com o indutor da fase inicial de uma resposta inflamatória de natureza, predominantemente, não-imune, que evolui para o processo de reparo ou de neoformação óssea ou dental, e não para a reabsorção como concluíram os autores. Portanto, defendo a tese de que a reabsorção dental ocorre, predominantemente, por mecanismo não-imune e dependente da alteração da concentração dos fatores de crescimento, normalmente expressos na dentina, via aumento da expressão e ativação de RANKr/RANKL, em células reabsortivas, decorrente da geração de citoquinas, fatores de crescimento e de outros fatores reabsortivos locais e sistêmicos; bem como, via mecanismo imune dependente da concentração de proteínas dentinárias associadas a LPS, removidas da raiz dental durante o movimento ortodôntico, trauma ou outros fatores. Enquanto a reabsorção dental “idiopática” pode ser causada por uma alteração genética, que altera a concentração de alguns dos fatores de crescimento para a dentina, capazes de ativar as células reabsortivas. Conclusões 1. Estratos de dentina hígida desmineralizada apresentam propriedade neoformadora óssea e cementária, podendo ser utilizados na terapia periodontal regenerativa; 2. Tais extratos induzem um processo reabsortivo inicial, necessário à liberação de fatores de crescimento e citoquinas da matriz extracelular, que, altera a concentração desses mediadores, tanto em osso quanto em dentina, evoluindo para o processo de reparo; 3. Em estados patológicos, na dependência da concentração, algumas poucas proteínas dentinárias, quando testadas isoladamente, podem apresentar atividade reabsortiva, via mecanismo não-imune; 4. Nesses estados, a atividade reabsortiva dessas proteínas é determinada por sua maior ou menor concentração no local de reabsorção, sem qualquer relação com sua maior ou menor antigenicidade; 5. Na dependência da concentração, algumas das proteínas dentinárias, quando testadas isoladamente e associadas a LPS, podem apresentar atividade reabsortiva, via mecanismo imune; 6. A reabsorção dental ocorre predominantemente por mecanismo não-imune dependente da concentração, via aumento da expressão e ativação dos receptores RANKr/RANKL, em células reabsortivas; 7. A reabsorção dentária observada durante o movimento ortodôntico, ou quando induzida por traumatismo, é causada, basicamente, pela ativação de mecanismos não-imunes induzidos pela força ativa; 8. A reabsorção dentária “idiopática”, possivelmente, decorre de uma alteração genética que - quando expressa -, altera a concentração local, normalmente expressa, dos fatores de crescimento para a dentina, ou de seus receptores, induzindo à reabsorção, via aumento da expressão e ativação de RANKr/RANKL em células reabsortivas; assim como a diferenciação de suas células progenitoras.
