Required Documents for Admission:

Birth Certificate
Social Security Card
Copy of Medicaid card
Immunization Records
Academic Records (if available)
Current medical records if there are
significant physical issues
Current psychiatric evaluation if there are
any mental health issues
IMPORTANT: 30-day supply of medications
New Admissions are allowed to
bring:















5-7 pairs of pants/ shorts/ cargo shorts/
capri pants/ sweatpants/ athletic shorts/
jeans
7 t-shirts or collared shirts (nothing
red/nothing with drug, alcohol, or gang
paraphernalia)
Neutral color jacket or sweater
Sleepwear (please bring only appropriate
items- no short shorts/nothing showing
cleavage)
One belt
One pair tennis shoes
Shower shoes
Underclothes and socks
Large bath towels
Face towels
Blanket
Mesh laundry bag
Vanilla VISA gift card
Toiletries (liquid soap, deodorant,
toothpaste, toothbrush, shampoo, lotion,
brush/comb*) *no metal brushes or
combs
Bible, AA/NA book, book (nothing to do
with alcohol, drugs, and/or violence)








Electronic devices of any kind
Cash
Any product containing alcohol
Weapons
Tobacco products
Drugs or Drug paraphernalia (if brought
onto OHA property charges may be
pressed)
Snacks
Food
Drinks
Makeup
Aerosol cans of any kind
Electric devices (blow dryers, curling
irons, electric razors)
Pillows
Baseball caps
Please Note:
We know that in today’s economy times
may be hard. Please strive to bring as many
items on the above list as possible, but DO
NOT feel that the admission cannot take
place if an item from the above list is
missing. We do not require a shopping trip
for any youth to be admitted.
