When We Are Closed
The out of hours service operates from 18:00 until 08:00 Monday to Thursday and from
18:00 Friday until 08:00 Monday.
If you require urgent medical attention outside the practice opening hours, please telephone the
Lothian Out-of-hours Service on 111.
Upon contacting the Out of Hours Service you will be asked for your name, address, telephone
number, doctors name and address and details of any medication that you are taking.
An experienced nurse will assess your symptoms and provide advice. Depending on the outcome
of this assessment by NHS 24, you will either be seen by appointment at one of the out-of hours
centres which are located across Lothian, or when necessary, in your own home. Sometimes a
doctor will call you back to discuss your problem further.
If you telephone the practice number out with opening times you will automatically be diverted
to the service.
Please note that this is not a walk in service.
The nearest Accident & Emergency Department to Haddington for adults is the New Royal
Infirmary, Little France, Edinburgh. Children under the age of 13 should be taken to the Royal
Hospital for Sick Children, Sciennes Road, Edinburgh.
Out-of-hours services are generally busy so please think carefully before asking to see a doctor
and only do so if you genuinely cannot wait until the surgery re-opens.
In a genuine emergency you should call 999. Chest pains and / or shortness of breath
constitute an emergency.
Appointments
Appointments can be made by telephoning the practice on (01620) 823183. You may consult any
doctor you choose but we would recommend that when possible, you try to see the same doctor,
as this enables us to get to know one another.
We always try to offer you an appointment on the day that you request, but it may be necessary
to wait if you would like to be seen at a certain time or by a specific doctor.
If you have a problem that will not wait for a routine appointment or the doctor of your choice,
please tell the receptionist that your problem is urgent. You will then be seen as an emergency by
the duty doctor.
Telephone Advice
If you would like to speak with a doctor, the best time to ring is before 10:00 or between 15:00
and 15:15. If the doctor is unavailable the receptionist will ask for your name, address, date of
birth, contact telephone number and reason for the call. This enables the doctor to prioritise his /
her telephone calls. The receptionist has been asked by the doctors to request this information so
please do try to help them by giving as much detail as possible.
If your call is urgent and cannot wait until the doctor is available, please ensure that you
advise the receptionist of this and they can direct your call to the duty doctor.
Interpretation & Translation Service
The practice complies with NHS Lothian’s policy to provide interpreters and translations when
requested by the service user. Please ask reception staff for further information.
Cancellations
If you cannot attend an appointment for any reason please inform us as soon as possible in order
for us to give the slot to someone else. You can also do this online by using the link on the
bottom of the Home Page.
Home Visits
Whilst we encourage our patients to come to the surgery, where we have the proper equipment
and facilities available, we do appreciate this is not always possible. In this respect, if you do
need a home visit, you can help us by calling reception before 10:00.
You can request a home visit if you are housebound or are too ill to visit the practice. Your GP
will only visit you at home if they think that your medical condition requires it and will also
decide how urgently a visit is needed. Please keep this in mind and be prepared to provide
suitable details to reception which will enable the doctor to schedule house calls
You can also be visited at home by a community nurse if you are referred by your GP. You
should also be visited at home by a health visitor if you have recently had a baby or if you are
newly registered with a GP and have a child under five years.
Sickness Certificates
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your
employer may however require you to complete a self-certification form (SC2) which is
available from your employer or on the HMRC website (www.hmrc.gov.uk/forms/sc2.pdf) .
Evidence that you are sick
If you are sick for more than seven days, your employer can ask you to give them some form of
medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate,
now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence
that you are sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist.
Your employer will decide whether or not this evidence is acceptable. If your employer has any
doubts, they may still ask for a medical certificate from your GP.
Statement of Fitness for Work - ’Fit Note'
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help
you return to work sooner by providing more information about the effects of your illness or
injury.
For more information see the DirectGov website
(www.direct.gov.uk/en/MoneyTaxAndBenefits/BenefitsTaxCreditsAndOtherSupport/Illorinjured/
DG_175850) (where this information was sourced)
Training practice
We have been approved as a training practice and may have attached to us doctors training for
general practice. These doctors are fully qualified and supervised by us. You may be offered an
appointment with them or be visited by them at home.
On occasions the practice may ask if your consultation can be video recorded for training
purposes. This will not be undertaken without your consent and no intimate examination will be
recorded. You may ask for the camera to be switched off at any time.
Repeat Prescriptions
If you are on long term medication, your doctor may arrange for you to obtain a repeat
prescription without being seen on each occasion.
Prescriptions may be requested by telephoning the practice between 10:00 and 16:00. You can
also order online by following the link at the top of this page.
Prescriptions may be collected from the surgery or from a Haddington chemist of your choice.
We would be very grateful if you would order all of your repeat prescriptions at one time as this
would greatly reduce our administrative workload.
Medication Reviews
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at
least once a year to review these regular medications and notification should appear on your
repeat slip.
Please ensure that you book an appropriate appointment to avoid unnecessary delays to further
prescriptions.
Please allow 48 HOURS for prescriptions to be processed and remember to take weekends and
bank holidays into account.
Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed.
Any problems please telephone the surgery.
PLEASE NOTE THAT WE WILL AUTOMATICALLY ARRANGE FOR
YOUR PRESCRIPTION TO BE SENT TO THE PHARMACY THAT WE
HAVE ON FILE FOR YOU.
IF YOU WISH TO CHANGE THIS PLEASE INDICATE IN THE OTHER
DETAILS BOX ON STEP 3 (sending your request) WHICH PHARMACY YOU
WOULD LIKE TO USE AND IF THIS IS A TEMPORARY OR PERMANENT
CHANGE.
PLEASE NOTE THAT THE COMMENTS BOX SHOULD ONLY BE USED
FOR INFORMATION RELEVANT TO YOUR REPEAT PRESCRIPTION
REQUEST. NO MESSAGES WILL BE ANSWERED

again, speak with the patient or the patient requires further treatment we will contact you.
Test results are given by the receptionists after authorisation from the doctors. Results will only
be given to the patient concerned unless consent has been given to release the information to
another person.
Blood Tests
Blood Tests
A blood test is when a sample of blood is taken for testing in a laboratory. Blood tests have a
wide range of uses and are one of the most common types of medical test. For example, a blood
test can be used to:



assess your general state of health
confirm the presence of a bacterial or viral infection
see how well certain organs, such as the liver and kidneys, are functioning
A blood test usually involves the phlebotomist taking a blood sample from a blood vessel in your
arm. and the usual place for a sample is the inside of the elbow or wrist, where the veins are
relatively close to the surface.
You can find out more about blood tests, their purpose and the way they are performed on the
NHS Choices website.
X-Ray
X-Ray
An X-ray is a widely used diagnostic test to examine the inside of the body. X-rays are a very
effective way of detecting problems with bones, such as fractures. They can also often identify
problems with soft tissue, such as pneumonia or breast cancer.
If you have a X-ray, you will be asked to lie on a table or stand against a surface so that the part
of your body being X-rayed is between the X-ray tube and the photographic plate.
An X-ray is usually carried out by a radiographer, a healthcare professional who specialises in
using imaging technology, such as X-rays and ultrasound scanners.
You can find out more about x-ray tests, how they are performed, their function and the risks by
visiting the NHS Choices website (www.nhs.uk/conditions/x-ray/Pages/Introduction.aspx) .
Registration
New Patient Registration
If you live within the practice area you can apply to join the practice list. (Click here to view
boundary map). Reception will ask you to provide your medical card or complete a patient
registration form and will also ask for proof of your address such as a utility bill or rent book.
Registration Form and Documentation Required for Registration.
You will also be asked to complete a new questionnaire with information on your health, details
of any allergies and medication that you may take. This data will be used until your medical
records are received by the practice. New Patient Questionnaire
All new patients are asked to arrange an introductory appointment with the practice nurse. The
practice nurse will perform a brief medical check and may ask for further details of any health
issues provided on your new patient registration form.
Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the
services on offer at GP Surgeries and how to access them. You can download the guide below.
A Patient Guide to GP Services (www.rcgp.org.uk/pdf/rcgp_iyp_full_booklet_web_version.pdf)
Temporary Residents
Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one
you can receive emergency treatment from the local GP practice for 14 days. After 14 days you
will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on
the local practice list and still remain a patient of your permanent GP. After three months you
will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do
not have to accept you as a temporary patient although they do have an obligation to offer
emergency treatment. You cannot register as a temporary patient at a practice in the town or area
where you are already registered.

When we have a duty to others e.g. in child protection cases anonymised patient
