SEE ALSO (available to BMA members only): http://bma.org.uk/practical-support-at-work/contracts/sessional-gps/locum-gphandbook A] Locum GP Induction Start with a walk around the Surgery premises: Fire Exits, Kitchen, Toilets, Reception, Administration Office – and introduce to all the Staff explaining all their job roles so the Locum will know who to ask for what. Show the Locum the location of emergency equipment: Defibrillator, Resuscitation kit, Oxygen, nebulisers, emergency medication. And who to ask for help delivering emergency care: Which other Doctors, Nurses or Health Care Assistants will be present whilst the Locum is on the premises. Supply a list of key codes, and advice on how to enter and leave the surgery premises if the surgery is closed: Not applicable to Locum Doctors at Hill View Surgery. Set up the computer system: So that the Locum can log in with their NHS Smart Card. [Both to the desktop PC and to the Clinical Database] Set up the desktop PC: So that the Locum GP has online access (desktop shortcuts) to any applications needed, e.g. Lab results, Choose and Book. Set up the desktop PC browser so that useful documents and online web links are available: (Some Practices will have this sort of information organised on a Practice Intranet system). B] Induction: getting started How to summon patients: Patients are normally summoned by the Doctors personally going to the Waiting Room and calling the patient by name. Computer Login procedures (and who to ask for help): All Locums have to log in with a Smartcard. The Smart-card number should be sent to the Practice by the Locum or the Locum Agency at least two weeks before they are due to work for the Surgery as we have to register each Locum individually. The Practice Manager or Reception Staff will be able to assist the Locum if any problems. Show the Locum GP: The “panic button” arrangement. The panic button can be initiated by pressing “control + shift + enter” at the same time. The Locum GP: Is likely to carry a mobile phone, and can provide the number to the Practice Manager. Acute prescribing: Advise re any in-house or locality formulary, and access to the local ‘traffic light’ database. If any queries the Locum can ask the Practice Manager or any of the staff who usually deal with prescriptions. LINK for practice policy if applicable Repeat prescribing: Locum GPs can sign repeat prescriptions if they are happy to do so. Prescribing clerks only print these off for signatures if there are issues left and regular GPs have authorised these. Locum Doctors at ****** Surgery will / will not be required to undertake Home Visits to patients except in extreme circumstances. This will include advice as to how to communicate clinical concerns, or alerts re follow up, to a GP colleague. Where are patient advice leaflets kept? Details of the Practice Complaints Policy: Locum GPs will need to be informed if they are subject to complaint and allowed access to (and fully anonymised copies of) all the relevant medical records and associated correspondence. They should be assisted in replying personally (alongside the Practice reply and via the Practice Complaints Manager) to the complaint. As a rule, complaints against Locum GPs should be handled “as if” the Locum GP was a Salaried GP employee. The Locum GP will need to take independent Defence Society (and LMC) advice. This will be followed by an appointment with a Doctor to review the ECG report where appropriate. X-Ray: USE ICE IF REGISTERED Locum Doctors will be requested to fill out the paper x-ray card to give to the patient. The telephone numbers to ring and book the x-ray can be supplied by Reception Staff. They do not have access to the ICE system. Blood tests: USE ICE IF REGISTERED Locum Doctors will be requested to complete the paper blood forms and ask the patient to go to Reception to make the blood test appointment. If taking blood themselves, Locum Doctors still need to complete the paper blood forms as they do not have access to the ICE system. MRI scans e.g. for prolapsed discs: USE ICE IF REGISTERED As above for Drect Access MRI (head, spinal knee) scans are available- see pathways intranet. Spirometry: The patient can book spirometry or spirometry and reversibility at reception. USS (NON-OBSTETRIC): UNDER AQP (Any Qualified Provider)- USE ICE IF REGISTERED The Locum Doctor is requested to complete the paper Ultrasound form for the patient to hand to Reception Staff to send off as they do not have access to the ICE system. OBSTETRIC USS: USE ICE IF REGISTERED Direct access Cardiac Echo: COMPLETE FORM Open access Endoscopy: COMPLETE FORM Exercise ECG testing (RACPC referral): COMPLETE FORM On site pregnancy testing e.g. for ectopics: Pregnancy testing kits are available in the Practice Nurses room. Paediatric urine specimens systems: By referral only to Paediatric Ambulatory Faecal Occult blood tests: Locum Doctor to complete relevant form and hand to patient to book appointment at Reception for blood test. Adult MSU: Locum Doctors are asked to complete the paper MSU request form and hand the sample and the form to Reception Staff who can process. G] Emergency admissions Procedure: For any possible emergency admissions, the Locum Doctor is asked to discuss the patient further with the regular Doctor at the Surgery before admitting the patient to hospital. Who to ask ‘in-house’ to discuss a possible admission: As above, the Locum Doctor needs to discuss any possible admission with the regular Doctor at the Surgery before making any admissions. Joint and soft tissue injections: As above IUD fitting: routine and emergency: Reception can add the patient to the Family Planning Clinic list. Depo contraception injection: Can be given by the Doctors. Dietician: By referral only Physiotherapy: A Physiotherapy form can be completed by the Doctor and patient. This will then be sent internally by the Reception Staff. Chiropodist: By referral only Booking with Midwife for antenatal care: The Reception Staff can book Midwife appointments for patients who are more than 8 weeks pregnant. Diabetic clinic: The Reception Staff can book patients into in-house Diabetic Clinics. Hypertension clinic: Appointments can be made with the Practice Nurse. Asthma clinic: Appointments can be made with the Practice Nurse. Wart clinic: Histofreeze appointments can be made with the Practice Nurse. Counselling: The patient can self-refer to Let’s Talk Wellbeing on 01159 560888 Clinical Psychology: By referral only Smoking cessation counselling: The patient can call New Leaf on 0800 389 7712 I] How to make referrals Who to ask about referral processes: Locum GPs should expect a GP Partner to read and ‘vet’ all their outgoing referrals, and if appropriate to re-reroute them ‘in-house’ or review the referral decision with the patient. This applies to all the following specialities unless specified otherwise below. Routine referrals are sent via a Clinical Gateway who then triage / signpost and ring the patients direct to arrange an appointment at the hospital of their choice. 2WW referral procedures: The Locum Doctor can dictate a letter or send a task to the Secretaries who will book the appointment whilst the patient is at the Surgery or afterwards if the patient has left. GUM: The patient can contact directly by ringing 01623 622515 and requesting the GUM Clinic. Termination of pregnancy: The Secretaries can supply the patient with a telephone number to make an assessment appointment with NUPAS – the Locum Doctor will need to enter relevant information on the patients screen e.g. LMP etc. so the form can be completed and faxed to NUPAS. Antenatal care and arranging first trimester scan: The patient can book a first appointment with the Practice Midwife at Reception when they are more than 8 weeks pregnant.
