Flowchart for Falls Referral Issue: March 2010 - Review March 2011 Person has history of falls or fear of falling Are they taking 4 or more medicines or any medications from “Risk List”? NB: combinations increase risk No Consider other risk factors and refer to appropriate service as required Yes Discuss with Falls Champion before referral Is the person resident in a care home? Yes No Who dispenses their prescriptions? Prescriptions dispensed by local pharmacy Give referral form and information to patient. Patient should visit pharmacy for Medicines Use Review (MUR) Prescriptions dispensed by Dispensing GP Complete referral form for GP appointment Contact NHS Norfolk Medicines Management Team via [email protected] or [email protected]
