Antibiotic Prophylaxis
Per AAOS & ADA Guidelines
Current as of August 2009
Antibiotic prophylaxis for patients with total joint replacements should be considered
prior to certain dental procedures as outlined below.
If
1.
You have had a Prosthetic Joint Replacement (i.e. Hip, Knee, Shoulder, Ankle)
AND
2. You will be having one of the following dental procedures:
a.
b.
c.
d.
e.
f.
g.
Dental Extractions
Root Canal or other endodontic instrumentation
Prophylactic cleaning of teeth where bleeding is expected
Intraligamentary local anesthetic injection
Dental implant placement or reimplantation of avulsed tooth
Periodontal surgery (surgery on or below your gums)
If clinical judgment anticipates significant bleeding in procedure
THEN
3. You should receive one of the following antibiotic regimens
a. Cephalexin, or Cephradine, or Amoxicillin
i. 2 grams 1 hour prior to procedure
b. If patient unable to take pills
i. Cefazolin- 1gram IV/IM 1 hour prior to procedure
ii. Ampicillin- 2 grams IV/IM 1 hour prior to procedure
c. If patient is Allergic to Penicillini. Clindamycin- 600 mg orally 1 hour prior to procedure
d. Allergic to Penicillin and can’t take pills
i. Clindamycin- 600 mg IV/IM 1 hour prior to procedure
*** No Second Doses are required for any of these dosing regimens
Antibiotic prophylaxis is not recommended for any of the following:
 Retained hardware after fixation of fractures or other orthopaedic
procedures





Fluoride treatments
Restorative dentistry (dental caries)
Adjustment of Orthodontic appliance (braces etc)
Taking oral impressions
Postoperative suture removal
