APPLICATION FOR SSI/ISID INFECTIOUS DISEASES FELLOWSHIP
 Instructions
To apply for the fellowship, please submit the following materials:
 A completed application form
 A current curriculum vitae
 A personal statement describing your career goals and objectives and how you
expect the fellowship to contribute to your professional development and to benefit
your home institution/country (1-2 pages)
 Two letters of reference from your home institution (e.g. hospital administrator,
clinical director, service chief, departmental chair)
 One letter of commitment from your home institution guaranteeing your position upon
completion of the fellowship
 Deadline
The deadline for applications is 1 April.
